| Description: | Facility ID#: 41 / 9815093 Inspection start date: 6/14/2024 Facility Name: Bay Vue Nursing (Last registered name) Installation Inspection Field Notes SITE VISITS: 6/14/2024 – Mark Trethewey on-site at 2:30PM. Installer: Unknown Activities: Upon Facility number review it was discovered that Tank #1 had been originally registered as “Hazardous Substance” and therefore did not populate to a regulated AST list. A site visit confirmed the tank was a regulated system (generator fuel). Upon arrival, it was discovered that a second gen-set had been installed. Although the install was observed by an unknown Manatee County representative, the Natural Resources Environmental Protection Division was not notified. Installations were not observed. Tank 1: Manufacturing info was not provided or available on the tank, system was registered in 2015 but appears to be older. Unknown install date. The tank appears to be UL-142 sub-base generator tank (EQ-969). There is no spill bucket attached, monitoring appears to be manual. The tank is in satisfactory condition with minor surface corrosion. Tank 2: Manufactured by: Advanced Manufacturing and Power Systems, Inc. 1965 Bennet Drive DeLand, FL 32724 386-822-5565 Model MH29522 UL D27750994 Manufacture Date: 11/6/2019. Unknown install date. 2400-gallon tank. Both are double-walled and set onto a concrete pad. Both have been put into service. Tank #2 still has vacuum on the interstice at -4. Overfill protection has not been established on either AST, if manual gauge ensure that no tank is filled beyond 90% capacity. FINANCIAL RESPONSIBILITY: Not provided. Provide Manatee County NRD with proof of Financial Responsibility indicating tank systems and Facility location. RELEASE DETECTION, as of 6/14/2024: a. Vacuum monitoring of the double-wall tank interstitial (TK2) manual monitoring (TK1). Manual overfill protection with gauged delivery on TK 1, High / low level gauge potential on TK2. b. Monthly visual inspections to be documented at least once a month not exceeding 35 days apart to include the tank exterior, spill containment and surrounding area. | ||||||||||
