Release detection: -Visual inspection of the sumps, dispensers/liners, and spill buckets. -Electronic monitoring of the tank interstice, dispenser liners, and STP sumps using a Veeder Root TLS 450. -Panel indicated: "All Functions Normal". -Audible/visual tested - appeared functional. -Alarm history and liquid status report printed and on file: No unexplained alarms noted. -Mechanical line leak detectors present with annual 3gph testing. Tanks (3): (2) 20,000 gallons and (1) 12,000 compartmented (Premium/ E0) double-walled Xerxes fiberglass UST’s with associated underground Ameron Dualoy LCX fiberglass piping. Tanks are equipped with: -(4) Vent lines present with caps. -(4) STP sumps inspected: appeared dry and intact. (5) STP heads appeared in good condition. *(2) STP heads in Regular sump. Sensors appeared to be positioned correctly. Secondary piping appeared open to sumps. Mechanical line leak detectors present. -(4) double-walled spill buckets inspected: appeared dry and intact. -(4) Spill bucket interstitial gauges inspected: not currently indicating a leak. -Fill lids appeared to be properly marked. -Overfill protection: flow shut-off valves present in fills. Additional overfill protection sensors present outside on vent lines near tank pad. Alarm tested and appeared operational. -Dual point vapor recovery present. No obvious signs of leakage noted. Dispensers: -(7) Dispenser/liners inspected: appeared dry and intact. -Shear valves appeared anchored. -Hoses/nozzles appeared in good condition. -Sensors appeared to be positioned properly within sumps. No obvious signs of leakage noted. Records: -Current registration placard posted onsite -Monthly monitoring logs reviewed and include: Visual inspections of spill buckets, dispenser liners, and STP sumps (in addition to electronic monitoring). Tank 1: 12,000 gallon double-walled Xerxes fiberglass UST to hold Diesel (EQ #200) UL# G-03538883 Tank 2: 20,000 gallon double-walled Xerxes fiberglass UST to hold Regular (EQ #200) UL# G-03538884 Tank 3: 20,000 gallon double-walled compartmented Xerxes fiberglass UST to hold Premium and Ethanol Free (EQ #200) UL# G-03538831 -Pressure was applied to the primary tanks during this inspection. Pressure readings were as follows: Tank #1: Start - 5 PSIG End - 5 PSIG Tank #2: Start -5 PSIG End - 5 PSIG Tank #3: Start - 5 PSIG End - 5 PSIG All manways and openings soaped- no bubbles noted. No change in pressure noted during inspection. -Entire tank exterior visually inspected. No damage or repairs noted. -Tanks were resting on pea gravel and clean soil. Anchoring Inspection: -Tanks were placed on pea gravel bed in excavation. -Pea gravel will be used as backfill material. -Proper spacing between and around the tanks according to manufacturer's specifications. -Tanks anchored using deadmen. Straps were provided by the manufacturer. -8 straps were used to anchor the compartmented tank. Straps were properly spaced. -Straps secured, cable ends directed away from the tanks. Turnbuckles were coated with mastic. Next inspection: Primary and secondary piping pressure test and hydrostatic test of the STP sumps and dispenser liners.

Primary and secondary piping was pressurized on 12/16/2022. Initial primary piping gauge readings were as follows: R: 50 PSI; P: 49 PSI; D: 49 PSI; E0: 46 PSI Initial secondary piping gauge readings were as follows: R: 4 PSI; P: 4 PSI; D: 4 PSI; E0:46 PSI -All entry boots were inspected. Pressure was observed for approximately 45 minutes onsite. No drop in pressure noted. -Some pressure released from all gauges at the conclusion of the inspection - appeared operational. -Piping primary and secondary pressure tests passed. Hydrostatic test of the Containment Solutions sumps (EQ #395) and Bravo B7000 dispenser liners (EQ# 727). -All STP sumps and dispenser liners were filled with water to above the last seam and penetration and the water level was marked on 1/17/2023. -Shear valves have been capped and were not tested at this time. -Water level appeared to have remained static. -No signs of leakage or bubbling noted. -Exterior penetrations on all STP sumps and dispenser liners appeared dry. -Hydrostatic test of the STP sumps and dispenser liners passed. Note: A survey drawing of installed tanks and underground integral piping, signed and sealed by a professional land surveyor or professional engineer licensed in the state of Florida, shall be maintained as a record in accordance with Rule 62-761.710, F.A.C. Note: Underground Storage System Installation and Removal Form for Certified Contractors must be submitted to EPC within 21 days.

Tank 1: 12,000 gallon double-walled Xerxes fiberglass UST to hold Diesel (EQ #200) UL# G-03538883 Tank 2: 20,000 gallon double-walled Xerxes fiberglass UST to hold Regular (EQ #200) UL# G-03538884 Tank 3: 20,000 gallon double-walled compartmented Xerxes fiberglass UST to hold Premium and Ethanol Free (EQ #200) UL# G-03538831 -Pressure was applied to the primary tanks during this inspection. Pressure readings were as follows: Tank #1: Start - 5 PSIG End - 5 PSIG Tank #2: Start -5 PSIG End - 5 PSIG Tank #3: Start - 5 PSIG End - 5 PSIG All manways and openings soaped- no bubbles noted. No change in pressure noted during inspection. -Entire tank exterior visually inspected. No damage or repairs noted. -Tanks were resting on pea gravel and clean soil. Anchoring Inspection: -Tanks were placed on pea gravel bed in excavation. -Pea gravel will be used as backfill material. -Proper spacing between and around the tanks according to manufacturer's specifications. -Tanks anchored using deadmen. Straps were provided by the manufacturer. -8 straps were used to anchor the compartmented tank. Straps were properly spaced. -Straps secured, cable ends directed away from the tanks. Turnbuckles were coated with mastic. Next inspection: Primary and secondary piping pressure test and hydrostatic test of the STP sumps and dispenser liners.

Description: 1/17/2023 BG/TIA Met onsite with Ishmael of Ishmael's Petroleum LLC working under the license of (PCC1256857) for the piping pressue test and hydrostatic testing of the STP sumps and dispenser liners. -Air test of the Ameron Dualoy LCX fiberglass piping (EQ #291). Primary and secondary piping was pressurized on 12/16/2022. Initial primary piping gauge readings were as follows: R: 50 PSI; P: 49 PSI; D: 49 PSI; E0: 46 PSI Initial secondary piping gauge readings were as follows: R: 4 PSI; P: 4 PSI; D: 4 PSI; E0:46 PSI -All entry boots were inspected. Pressure was observed for approximately 45 minutes onsite. No drop in pressure noted. -Some pressure released from all gauges at the conclusion of the inspection - appeared operational. -Piping primary and secondary pressure tests passed. Hydrostatic test of the Containment Solutions sumps (EQ #395) and Bravo B7000 dispenser liners (EQ# 727). -All STP sumps and dispenser liners were filled with water to above the last seam and penetration and the water level was marked on 1/17/2023. -Shear valves have been capped and were not tested at this time. -Water level appeared to have remained static. -No signs of leakage or bubbling noted. -Exterior penetrations on all STP sumps and dispenser liners appeared dry. -Hydrostatic test of the STP sumps and dispenser liners passed. Next inspection: Final Inspection.

Release detection: -Visual inspection of the sumps, dispensers/liners, and spill buckets. -Electronic monitoring of the tank interstice, dispenser liners, and STP sumps using a Veeder Root TLS 450 (EQ #724). -All sensors tested during this inspection and passed. -Panel indicated: "All Functions Normal". -Audible/visual tested - appeared functional. -Mechanical line leak detectors present with annual 3gph testing. Tanks: (2) 20,000 gallons and (1) 12,000 compartmented (Premium/ E0) double-walled Xerxes fiberglass UST’s (EQ #200) with associated underground Ameron Dualoy LCX fiberglass piping (EQ #291). Tanks are equipped with: -(4) Vents present with caps. -(4) Containment Solutions sumps (EQ #395) inspected - appeared dry and intact. -STP heads appeared in good condition. -Sensors positioned correctly. -Secondary piping appeared open to sumps. -Mechanical line leak detectors present (EQ #335) -(4) EMCO Wheaton A1005-518 series spill buckets (EQ #695) inspected - appeared dry and intact. -(4) Spill bucket interstitial gauges present - appeared dry and intact. -Fill lids properly marked. -Overfill protection: EMCO Wheaton overfill prevention valves present in fills (EQ #414). -Dual point vapor recovery present. No obvious signs of leakage noted. Dispensers: -(7) Bravo B7000 dispenser liners (EQ# 727) inspected - appeared dry and intact. -Liners were properly sized to contain overhead equipment (conversion pans present). -Shear valves appeared anchored. -Hoses/nozzles appeared in good condition. No obvious signs of leakage noted. Records: -Current registration placard not posted on site, however, payment of registration placard has been made. Obtain registration placard, maintain original on site and submit a photo of the placard posted onsite to EPC. -No Class A, B or C Operator training certificates were available for review. Obtain Class A, B and C operator training certificates and submit copies of certificates to EPC upon completion. -Monthly monitoring inspections must be performed every calendar month within 35 days and include: Electronic monitoring of tank interstice, and sumps. Alarm panel status checks. Alarm status printouts. Alarm history report printouts. Biannual visual inspection of sumps and dispenser liners. Visual inspection of electronically monitored components must be performed every 6 months and recorded in logs. Alarm history report and liquid status must be printed monthly. Inspections must be performed once a month, not to exceed 35 days in between. Note: A survey drawing of installed tanks and underground integral piping, signed and sealed by a professional land surveyor or professional engineer licensed in the state of Florida, shall be maintained as a record in accordance with Rule 62-761.710, F.A.C. Note: Underground Storage System Installation and Removal Form for Certified Contractors must be submitted to EPC within 21 days.
