Description: | Met onsite with Forrest Ward (Lead Operator) with Fairpoint Regional Utilities for this routine tank compliance inspection. New Aboveground Storage Tank (AST) System installed in March of 2023 per registration. Our department was never notified of the install - violation issued and resolved. Equipment Installed: TANK: The tank is registered as Tank #2. Tank #2 is a 917 gallon sub-base AST supplying fuel to an emergency generator (Fidelity Manufacturing FL EQ# 497 - Generac Generator). The tank appears to be in good condition with no surface rust or evidence of leaks. The tank is bolted down to an approximate 10 to 12 inch concrete slab. The tank system is equipped with a normal vent and 2 emergency vents. PIPING: Synthetic supply and return fuel lines. The fuel lines run directly from the tank-top to the generator. The fuel lines appeared to be in good condition with no evidence of leaks and visible cracking. SPILL/FILL CONTAINMENT: The tank is equipped with an approximate 5 gallon spill containment bucket/box (factory equipped). The spill containment was clean and dry at time of this inspection. RELEASE DETECTION: Rupture Basin Sensor & Corresponding Alarm (factory equipped). Alarm panel is located near the spill containment bucket. Alarm panel is both visual and audible. OVERFILL: High fuel level alarm (factory equipped) and Rochester fuel level gauge (FL EQ# 901). Monthly visual inspection are being conducted by Fairpoint Regional Utilities personnel. Annual Operability Testing of the rupture basin sensor & alarm is overdue - violation issued. Annual Operability Testing of the designated primary overfill device is overdue - violation issued. See violation section of inspection for corrective actions. Description: | TYPE OF INSPECTION: After-the-Fact Aboveground Storage Tank (AST) Installation Inspection. FACILITY OWNER: Fairpoint Regional Utility Systems, Inc FACILITY CONTACTS: Jeff Cringler, jcringler@hnws-fl.com; Donna Lupola, dlupola@hnws-fl.com AST CONTRACTOR: Culpepper Construction Company, agreen@culpeppercc.com EQUIPMENT INSTALLED: Tank was registered as Tank #2. TANK 2: 917-gallon Double-Wall (DW), steel, Sub-Base, Fidelity Manufacturing, LLC , Aboveground Storage Tank containing diesel fuel to power an emergency generator for a water well system (EQ-497). SPILL CONTAINMENT: Fidelity Manufacturing, LLC 7.5 Gallon Spill Containment for ASTs installed by Fidelity Manufacturing, LLC that came equipped from the manufacturer for both tanks (EQ-778). PIPING: Piping consists of synthetic fuel supply and return hoses that connect directly to the engine from the tank top. RELEASE DETECTION / LEAK DETECTION: FPI Sensors International, Leak Detection Switch (EQ-817). OVERFILL PROTECTION: FPI Sensors International, High Fuel Switch (EQ-817), Morrison Brothers, Overfill Protection Valve (EQ-858) and Rochester Fuel Level Gauge (EQ-901). Our office was not notified of this installation until after the installation was completed. Per the tank registration, Tank #2 was registered as installed March 2023. The facility was contacted and able to provide a work notification, site diagram, FDEP Approved EQ# List and financial responsibility. The tank is exempt from the wellhead letter due to the generator system providing emergency services for the well system. Tank #2 is 917-gallon, Double-Wall (DW), steel, sub-base, Fidelity Manufacturing, LLC, Aboveground Storage Tank (EQ-750). Tank #2 came equipped with a spill/fill box from the manufacturer (EQ-778). The tank is equipped with a FPI Sensors International, Leak Detection Switch (EQ-817) for leak/release detection and with FPI Sensors International, High Fuel Switch (EQ-817), Morrison Brothers, Overfill Prevention Valve (EQ-858) and Rochester Fuel Level Gauge (EQ-901) for overfill protection. This tank system is also equipped with a primary vent and two emergency vents. For piping, this system is equipped with synthetic fuel supply and return hoses that connect directly to the engine from the tank top. The tank is bolted down to an eight-inch-thick concrete pad. The tank’s interstitial, interstitial switch, overfill high level alarm, overfill prevention valves, fuel level gauge and spill/fill box were not tested prior to the system being placed into service. One of the facility representatives scheduled Taylor Power to test the tank and tank system equipment. Once on-site, they were notified that the company could not perform the required testing mentioned above. On 09/09/2024, the facility representative reached out to Cummins MidSouth for a quote to perform testing. Violations for system testing not being performed prior to placing the tank in service are be issued to meet FDEP Installation requirements. Please note that the Primary Overfill Device and the Tank’s Interstitial Alarm (Rupture Basin Alarm) are to be tested annually after the full system is tested. The annual testing records are to be kept on file with the facility and provided to the inspector from the last inspection to the next Routine Tank Compliance Inspection, with up to 3 years of annual testing being available for review. Also, monthly visual inspections for tank exterior and all tank related equipment will be required to be documented each month, not to exceed 35 days and the records provided to the inspection at the time of Routine Tank Compliance Inspections.
