A copy of this inspection report and compliance assistance offer letter will be emailed to Scott Janashak at Sjanasha@circlek.com, Kristen Sherman at ksherman@circlek.com, and Christopher Bavuso at Christopher.Bavuso@circlek.com. Tanks: Tank #1R1, Tank #2R1, and Tank #3 are 10,000 gallons Xerxes Corporation, Xerxes Double Wall Tanks (EQ-200). Tank #1R1 contains Regular Unleaded, Tank #2R1 contains Premium Unleaded, and Tank #3 contains Vehicular Diesel. All three tanks were installed July 1987. Piping: Single Wall 2-inch rigid FRP Amero with Red Jacket FX-1V, Mechanical Line Leak Detectors (EQ-373). Dispensers: S. Bravo Systems, Inc. B-8000 Dispenser Liner (EQ-300). The dispenser’s sumps were mostly clean and dry at the time of inspection. The boots appeared to be in good condition. The meter in Dispensers 1/2, 5/6, and 7/8 appeared to be weeping -Violation Issued. STP Sumps: Xerxes Corporation, Submersible Turbine Pump Sumps (EQ-397). The STP sumps were mostly clean and dry at the time of inspection. The boots appeared to be in good condition. The sensors were in the correct position. The Diesel STP Sump contained liquid -Violation Issued. No access available to the Premium STP Sump -Violation Issued. Spill Containments: Franklin Fueling Systems, 705-550 double-walled spill containment (EQ-757). All the Spill Containments contained liquid -Violation Issued. Overfill Prevention: Drop Tube Device. Regular Overfill Device Failed Operability Testing -Violation Issued. Release Detection: The tanks interstitial sensors and the STP Sump sensors are being monitored by a Veeder-Root USTMAN SIR Version 95.2B System (EQ-458). Records: All facility records were received, including associated tank component testing, monthly visual inspection with electronic release detection ATG printouts, and financial responsibility forms. The placard was also posted at the facility. See Violation Section for Corrective Actions.

Description: TANKS: Tank 1R1, 2R1, and 3 are fiberglass, double wall 10000 gallon USTs that were installed on 07/01/1987. Tank 1R1 contains unleaded regular gas, tank 2R1 contains unleaded premium gas, and tank 3 contains vehicular diesel gas. STP SUMPS: The accessible STP sumps were clean and dry at the time of the inspection. The diesel STP sump was inaccessible due to metal rim for lid being bent, preventing it from opening - violation issued. No testing was received for integrity testing for the STP sumps - violation issued. SPILL CONTAINMENT: The double-wall spill containment buckets and vapor recovery buckets, except the premium spill bucket, had excessive liquid present at the time of inspection - violation issued. OVERFILL PREVENTION: The facility uses drop tube for overfill prevention DISPENSERS: The dispenser sumps were mostly clean and dry at time of inspection with sensors position correctly. The hose and nozzles appeared to be in good condition at the time of inspection. No testing was received for integrity testing for the dispenser sumps - violation issued. RELEASE DETECTION: The tank STP sump sensors and dispenser sensors are monitored by a Veeder Root Automatic Tank Gauge (ATG). The spill buckets' interstitial gauges were operational. The interstitial gauge for the unleaded regular and diesel spill bucket showed liquid in interstitial space - violation issued. Release Detection is continuous electronic monitoring, using liquid sensors, of three UST interstices and STP sumps with a Veeder Root TLS-350 ATG. All PCW was removed during this inspection by PC Mechanical. The STP sumps and dispenser liners were mostly dry. Description: Annual Tank Compliance Inspection conducted 9/7/2011at 10:00 am: This facility's UST System appeared mostly clean, dry and functional for this inspection. All three (3) fillport spill buckets had excessive water in them (see photo) due to the recent strong rainy weather associated with Tropical Storm Lee - not a violation at this time but this water should be removed and disposed of properly as soon as possible. The Diesel fillport's drop tube has a rubber gasket out of place (see photo). This gaket should be realigned or replaced to prevent liquid access into the diesel tank. One of the EMCO Sump Traffic Lids has been cracked and may no longer support the weight of a vehicle. All Shear Valves and brackets were found tight and secure. The Placard, Financial Responsibility, System Tests, Release Detection Response Level, and Monthly Electronic and Visual Inspections are up-to-date and available for this inspection. The Veeder Root System does not display any sensor alarms, however, the System Status Report has a T 2: Probe Out warning that should be investigated.. Release Detection is continuous electronic monitoring, using liquid sensors, of three UST interstices and STP sumps with a Veeder Root TLS-350 ATG. The STP and Dispenser Sumps are visually inspected monthly. The Sump Pumps are equipped with Mechanical Inline Leak Detectors that will provide automatic pump shut-off if a piping leak occurs.

Description: Inspectgfion 02/15/2008. Excess water in 2 of 3 STP Sumps and 2 of 3 Spill buckets from rain event three days ago...needs removal. The only dry spill containment has an obviously broken clamping ring at its base, almost surely the proximal cause of a leak of unknown duration. It quite probably resulted from uneven tightening at installation. Dispenser sumps are dry and clean. No changes to Emergency Preparedness data (phone # or number of fueling positions.) --JSS Release Detection is Ustman SIR version 95.2B with tank tightness test every three years, annual test of Line Tightness, and Red Jacket FX-1V mechanical Line Leak Detectors (EQ-373) on each STP. All Sumps and spill containments are visually inspected.

Rule 62-761.610(1)(b) covers release detection for Category-C UST systems. Rule 62-761.610(2) deals with release detection for Category-A and B UST systems. Rules 62-761.610(3)(a)1, 62-761.610(3)(a)2, 62-761.610(3)(b) & 62-761.610(3)(c) deal with all piping release detection.

Comments: Installation - verify that the line leak detector has been installed during the final installation inspection. Compliance - check to see that the unit is present. Examine the annual function test data - a pass alone is not sufficient. The tester must demonstrate 3.0 gph discharge detection for the mechanical (flow restrictor). Note that the electronic (auto shut off) line leak detector does not need an annual test, but must still be able to detect a 0.2 gph leak after it has been tripped at 150% line operating pressure. Replacements - unit must be tested out of the box - function must be proven. 3.0 gph = 185.4 ml/min (To convert milliliters/minute to gallons/hour, multiple the milliliters by 0.0162162; or, 1 gph equals 61.8 ml/min). Values greater than 186 ml/min fail. Values less than 185 ml/min mean that the line leak detector will activate at a more restrictive rate.
