Description: New Violations: The walls and floor of the concrete secondary in the fuel truck dispensing area have cracks in them. Seal the cracks with an approved sealant and call for a re-inspection. Some of the dispenser pans and the remote fill containment are holding some product. It should be removed. Dispenser ½ has a shear valve that is not properly secured. Secure the valve and call for a re-inspection. Comments: Six SW steel ASTs with remote fill inside concrete secondary containment. There is above ground steel pipe suction system inside the containment transitioning to DW pressurized underground pipe to the dispensers with the exception of the off road dispenser. Hoses in good condition dispenser pans look tight, several are holding some product. There is also a top fill dispenser area for filling fuel trucks. This dispenser area has some cracks in the walls and floor that must be sealed. Visual monitoring of ASTs, pipe and secondaries. The horizontal ASTs have clock gauge monitoring for product level; the vertical ASTs have Moormann gauges. The ASTs share a sealed concrete secondary containment with a closed drain. The ASTs, piping and secondary are in good condition. The current registration placard was posted. The insurance policy, Certificate of Financial Responsibility and RDRL were in the log. Inspections were on time, but details of the inspections were not noted. A facility representative signed the attached notes after consultation. Discharge Information: EDI completed score 77 This report will be mailed via USPS. A non compliance letter citing the violations accompanies this inspection report; please find it attached. Please provide our office with documentation confirming the resolution of the cited violations within thirty days of receipt of this inspection report and attached non compliance letter. Description: Comments: After receiving information of a discharge I visited the site 20Oct14 to do a discharge inspection. As told to this inspector by Mike Doerr, who was told by Sean Fugate, as was reported by the truck driver: According to a truck driver: At approximately 0400 on 1Oct14 the driver saw evidence of a discharge and reported this to his employer, Sean Fugate. An unknown driver pulled up to a self service dispenser and started fueling his truck. He set the lock on the nozzle to continue fueling and was not attending when either the nozzle fell out of the tank and continued dispensing fuel, or the nozzle didn¿t shut itself off after the tank was full. This driver eventually discovered the discharge, stopped it and drove away. The reporting driver arrived to fuel his truck some time afterward. It rained that night and into the day. It is unknown when this driver told Sean Fugate. Mr Fugate told Mike Doerr at approximately noon that day. Mr Doerr reported this to his consultant, George Foster of Creative Environmental Solutions either later that day or the following day. Mr Doerr left the reporting of the discharge to DEP to Mr Foster. He said it didn¿t immediately occur to him to report to DEP. Note that the requirement for reporting a discharge is within 24 hours or the next business day. Creative Environmental Solutions notified the State Watch Office who notified the Office of Emergency Response on 13Oct14. The Office of Emergency Response will track and review until further notice. Creative Environmental Solutions submitted a Discharge Reporting Form to DEP on 13Oct14. Creative Environmental Solutions will be overseeing the excavation and submit a Source Removal Report to the Office of Emergency Response within 60 days of completion. It¿s a short way, 40-60 feet down a definite paved, mostly impermeable slope to the ditch by SW 18th Ave. The ditch continues to slope down toward the south. By the time Mr Doerr found out about the discharge, the fuel had completely adsorbed into the soil. The pavement had been washed, and the fuel washed into the soil by the rain. At this time there is dead grass showing the path of the fuel terminating in a patch of sand. I stepped off about 60 yards long and 2-3 yards wide. The estimation of 100 gallons spilled was derived from the amount of fuel dispensed. Discharge Information: EDI score 77 completed This report will be sent via USPS. Promptly empty secondary containment of liquids. 1 dispenser is missing a shear valve braket bolt. Replace the bolt. The offroad diesel dispenser is not sealed around in- pipe. Seal around the pipe. The offroad diesel fillport is green. Color codes are as follows: Yellow for road diesel or yellow with a blue stripe for off-road diesel. It would be helpful to number ASTs and dispensers, so they could be referred to more easiliy. Comments: Shop-fabricated AST dike field areas must have met these standards by January 1, 2000. Category-A and B field-erected AST dike field areas need to meet containment standards in .501(1)(e) and .501(3)(c) by January 1, 2005. Concrete secondary containment must meet one of three standards: 1) the concrete must be poured to ACI standards; or 2) a liner applied (either from the approved equipment list or applied to NACE/SPCC standards); or 3) the containment must be designed by a PE. Facilities must provide documentation that the concrete was installed according to ACI standards, if this option is used. Note that ACI concrete must be mixed according to a formula which involves a monolith pour. Applied liners must be compatible to the product stored. For example, coal tar epoxies won't work with petroleum products. Look at the manufacturer's specifications for limitations on the product to be used. The PE must be registered in the State of Florida. See rule 762.701(1)(c)1 for upkeep and maintenance issues.

The secondary containment unit is severely compromised at the west end. Furthermore it is not adequately sealed. Please repair the damaged west end, thoroughly clean the entire unit. Patch all the cracks then reseal it using F D E P approved sealant. 2). The dispenser containment for pump # 4/5 is holding some product (about one inch).
