On December 27, 2014, the Contractor performed a pre-hydro-test of the existing spill buckets and it was reported that all three failed (written documentation was not provided). The Inspector did not witness the pre-hydro-testing. As a result of the failed pre-hydro-testing, the Owner Operator submitted an Incident Notification.
