A disposal manifest & work order should be supplied showing fuel was removed from all of the tanks. 2) The tanks must be properly registered as 'out of service', using the FDEP registration portal: https://www.fldepportal.com/DepPortal/go/home 3) Vent lines should remain open and functional. 4) Fill ports must be locked. A picture should be supplied showing the tanks are locked. 5) Storage tank insurance must be current. 6) At least once every 12 months, check the tank interstice and liquid level in the storage tanks & document (unless the tank system contains no regulated substances). Message: Hello Philip, Thank you for the documentation and LCR. Attached is a in In-Compliance’ letter. However, the facility registration needs to be updated to show that the Diesel tank (Tank #6) no longer has a ‘Ball Check Valve’ and now has ‘Flow Shut-off’. Also, as of October 13, 2018, all containments in contact with soil need to be tested for integrity (fill ports’ test received, thank you). So you need to have the dispenser and piping sump containments integrity tested. Description: Scott Janashak, First Coast Energy, 904-955-0399, sjanashak@firstcoastenergy.com, called me at 1006 on Jan 3, 2013, to report a fuel spill from a tanker truck during delivery. It's possible that the delivery driver was in the cab of the truck and not overseeing the delivery when the spill occurred (due to rain). He did not know how much fuel was spilled. I asked that he submit an INF and any documents related to the cleanup such as disposal manifests. He said that he would email them to me. He also said that he planned to sample the nearby well and would probably need to replace some asphalt. I arrived on site at approximate 1200 on Jan 3, 2013. The store manager, Judy, said Scott had already been to the site and had left. She stated that the spill occurred on Jan 1, 2013, at the RUL fill port. There was a puddle of pcw near MW#4 on the eastern end of the asphalt and petroleum staining on the asphalt from the spill (area measured about 30' x 10'). There is a port labeled as an "observation well" that was covered by the puddle of pcw. This is not identified on other site diagrams. Update: Received an email from Scott at 1537 on Jan 3, 2013, with INF attached. Email states "There will be activity at this location tomorrow if you or Jackie Perry want to come out. As we obtain lab results, I will keep you in the loop". Asphalt should be removed by 1100 and soil screening will begin.

Significance: SNC-B Status: Closed
Criteria ID: 1062
Rule: 62-761.600(1)(d)
Comments: Mark this as out of compliance if any release detection method has not been monitored as required. Look for evidence that the release detection system is not being properly monitored. Examples include: blocked access to monitoring wells; ATG in alarm; ATG panel hidden from view; ATG tape spool empty; buried or submerged sampling ports; corroded locks; or no dispenser keys. Be aware that manual tank gauging requires weekly sampling. Electronic line leak detectors may be wired into an ATG with a memory of test cycles. Facilities with static/periodic ATG systems are required to provide one passing test per product per month. This method has a minimum fuel capacity requirement. Facilities with continuous electronic leak detection ATG systems (e.g., Veeder-Root CSLD) are required to provide one passing test per product per month, although the system must be in daily operation with monthly summary logs. This method has no minimum fuel requirement. If there is a record keeping problem, see rule 62-761.710(2).
05/11/2010 Violation Closed Leigh, Michelle M.
Significance: Minor Status: Closed
Criteria ID: 1115
Rule: 62-761.700(1)(c)1.
Comments: Facility equipment shall be maintained to a degree to allow you access. Water in spill containment units, especially above the fill cap shall be removed. Water in any sumps and dispenser liners above the piping opening/penetration (invert) shall be removed. All regulated substance shall be removed from any of the specified system components. Regulated substances: sheen, free product, mixed product water. Petroleum contact water shall be disposed of in accordance with the Department Hazardous Waste Section's Ch. 62-740 F.A.C., "Petroleum Contact Water".

Comments: Mark this as out of compliance if you observe a needed repair that hasn't been completed, and the potential for a release exists. Examples of components that would need repair include (but are not limited to) leaking dispensers, cracked spill containment, improperly anchored shear valves, or any compromised containment component. Upon the detection of one of the above conditions, has the facility initiated and completed repairs?

Comments: Mark this as out of compliance if you observe a needed repair that hasn't been completed, and the potential for a release exists. Examples of components that would need repair include (but are not limited to) leaking dispensers, cracked spill containment, improperly anchored shear valves, or any compromised containment component. Upon the detection of one of the above conditions, has the facility initiated and completed repairs?

Description: 2008.03.11.Annual. Placard and RDRL are current and posted. The Simplicity alarm sounds when tested. Fills are marked and drops are clear. Vapor recovery is in good working order. The electrical conduit entry boot in the regular unleaded sump is split. See picture. Dispenser liners are dry, shear valves are anchored, and boots appear to be in good shape. There is also a 550 gallon kerosene tank and dispenser which is in correct working order as well as a waste oil tank with release detection available for inspection.
