No work currently in progress. Tanks were not observed off loaded and are staged on Styrofoam sheeting. Tanks delivered 3/19/2020. Activities: 3/30/2020 – Mark Trethewey on-site at 8:30AM. Dewatering and excavation in progress. 3/30/2020 – Mark Trethewey on-site at 12:30PM. Tank #A has been set in place. Ameron LCX (EQ-291) piping has been installed. Pressure was introduced to the primary pipe 4/30/2020 and continues to hold as follows: First primary gauges: Regular set 50 / now 50. Premium set 50 / now 41. Diesel set 50 / now 51. Soap testing was performed. It appears that the primary lines are tight at this time. OPW Flexworks dispenser liners and sumps (EQ-368) were also hydro-tested. Water was introduced 5/1/2020 and all are currently holding at the mark. All systems appear tight at this time. 5/4/2020 – Mark Trethewey on-site at 4:00PM. Ameron LCX (EQ-291) piping has been installed. Pressure was introduced to the secondary pipe this morning and continues to hold as follows: Regular set 5 / now 5. Premium set 5 / now 6. Diesel set 5 / now 5. Soap testing was performed. It appears that the secondary lines are tight at this time. 1/21/2022 – Mark Trethewey on-site at 1:00PM. Final installation inspection performed. NOTE: Facility has been closed since last site visit. Piping test boot valve stems in the STP sumps were open to permit interstitial monitoring. Fill covers were properly marked, verified that drop tubes were installed. Interstitial gauges were green, showing no breach of integrity. Inspected Dispensers 1 through 8, All shear valves are rigidly anchored. A copy of the start-up tank, line precision tightness tests and test results of the MLLDs. has been received. Please note that the following documents are due as follows: 1. Updated Storage Tank Registration Form is due within 10 days. 2. UST Installation and Removal Form for Certified Contractors is due within 21days. FINANCIAL RESPONSIBILITY: Valley Bank, Letter of Credit; 5/31/2020 - 1/31/2022. RELEASE DETECTION, as of 1/25/2022: a. Monthly visual inspections of the Tank interstices, STP sumps, dispensers, dispenser liners, and spill buckets. b. Mechanical line leak detectors. A Complete Closure Report will be required within 60 days. Activities: Facility is removing old tanks currently Out-of-Service. Removal is being performed along with install of new systems. New tanks are in place, dispenser liners will be removed as part of install. All piping was rinsed to tanks prior to removal. 4/1/2020– Mark Trethewey on-site at 8:00 AM. Tanks were rinsed yesterday by Monark Environmental. Four (4) BuffHide coated steel, underground petroleum storage tanks are being removed. The tanks had been upgraded on 4/24/2009 with PetroFuse secondary containment to meet double-walled requirement. The 4000-gallon Diesel tank removal was observed first. Excavation was backfilled immediately to avoid damage to nearby utility pole. Three remaining 6000-gallon tank removals were observed (Premium, Regular and Regular Slave). Areas were backfilled after each removal to allow access to the next tank. All four tanks are in good condition with no apparent damage. Groundwater was not encountered due to pumping activities. All tanks were staged for disposal to Plant City. Sumps, piping and concrete were segregated for disposal in landfill. 4/2/2020– Mark Trethewey on-site at 8:00 AM. Tanks are being loaded for transport and disposal. 4/13/2020– Mark Trethewey on-site at 8:00 AM. Work is on hold. Removal of dispenser liners and any remaining piping will be performed when installation continues. CLOSURE INSPECTION ONLY. Certain records and equipment associated with routine compliance inspections were not checked. Please note that the following documents are due as follows: 1. An updated Storage Tank Registration Form is due within 10 days. 2. Underground Storage Tank Installation and Removal Form for Certified Contractors is due within 21 days. 3. Comments: The breach of integrity test is intended for closed interstices only. A closed interstice has no unsecured or unsealed opening to the outside. Double-walled piping without a boot connector, or if the boot has a bleed valve (for tightness testing), is considered an open interstice. The breach of integrity test may be performed by using at least one of the following methods: a. A continuous hydrostatic system b. A continuous vacuum system c. Testing of the interstice for liquid tightness in accordance with manufacturer's installation instructions; or d. Another method in accordance with subsection 62-761.850(2). How the breach of integrity test is performed or to what degree can depend on the depth to groundwater at each facility. When the tank and or piping are fully submerged, the lack of water intrusion into the secondary containment may be considered a verification of its integrity. However, when total submersion is not the case, one of the rule stipulated options must be used. The breach of integrity test should not be confused with a precision tightness test, which tests the primary tank or piping only. The breach of integrity test will test the secondary and primary shells, but is not a precision test.

Examine the annual function test data - a pass alone is not sufficient. The tester must demonstrate 3.0 gph discharge detection for the mechanical (flow restrictor). Note that the electronic (auto shut off) line leak detector does not need an annual test, but must still be able to detect a 0.2 gph leak after it has been tripped at 150% line operating pressure. Replacements - unit must be tested out of the box - function must be proven. 3.0 gph = 185.4 ml/min (To convert milliliters/minute to gallons/hour, multiple the milliliters by 0.0162162; or, 1 gph equals 61.8 ml/min). Values greater than 186 ml/min fail. Values less than 185 ml/min mean that the line leak detector will activate at a more restrictive rate.

The single-wall Ameron fiberglass piping from the piping transition sump to the tank STP sumps/culverts will be removed and replaced with Ameron DW Coaxial 3000/LCX fiberglass piping (EQ-291). Water had been added yesterday afternoon to the T1R1, T3R1, and T4R1 STP sumps and piping transition sump up to a marked point above the highest entrance boot. The levels were unchanged at the time of this inspection. The new Western Fiberglass sumps did show some leakage but were repaired prior to arrival on site. It appears that all secondary containment sumps are tight at this time The interstice of the double-wall Emco Wheaton spill buckets were vacuum tested at -5¿ Hg and held within +0.5¿ Hg for 1 minute. The Regular, Regular Slave, and Premium buckets passed but the Diesel would not hold vacuum. The inner bucket will be replaced and, in the interim, the Diesel bucket will be considered to be single-walled. Piping and tank backfill is clean soil.

Comments: Installation - verify that the line leak detector has been installed during the final installation inspection. Compliance - check to see that the unit is present. Examine the annual function test data - a pass alone is not sufficient. The tester must demonstrate 3.0 gph discharge detection for the mechanical (flow restrictor). Note that the electronic (auto shut off) line leak detector does not need an annual test, but must still be able to detect a 0.2 gph leak after it has been tripped at 150% line operating pressure. Replacements - unit must be tested out of the box - function must be proven. 3.0 gph = 185.4 ml/min (To convert milliliters/minute to gallons/hour, multiple the milliliters by 0.0162162; or, 1 gph equals 61.8 ml/min). St. Petersburg, FL Joe Samon: (727) 423-2131 A partial piping upgrade of the gasoline dispensing area under the canopy and the stand-alone diesel dispenser is being completed which includes the following: double-wall (3000/LCX) Ameron fiberglass piping (EQ-291), Petroleum Containment dispenser liners (EQ-203), and a Petroleum Containment transition sump (EQ-203). The existing single-wall (3000/L) Ameron fiberglass piping that runs from the tank field will remain and terminate in the transition sump. Backfill will be clean sand. The primary piping had been installed along with the dispenser liners and a transition sump. Air pressure at 50 psig was introduced to the primary lines at 8:00 AM and was holding within 5% at the time of this inspection. The piping joints and shear valves were soap tested by the contractor with no bubbles observed. It appears that the primary piping is tight at this time. The ¿stand-alone¿ diesel dispenser had been removed and a piping transition sump was installed in the same location. A new diesel fueling position was located at the Northwest dispenser #1/2. Dispenser #1/2 will now have four (4) products: diesel, regular, plus, and premium. 05/6/2008 ¿ Mark Trethewey on-site at 11:45 AM The secondary piping clamshell fittings had been installed for the auto dispensers in the front of the store. Air pressure at 5 psig for the Premium, Regular and Diesel had been introduced this morning at approximately 8:30 AM and was holding within 5% (6 psig, 5 psig and 6 psig respectively) at 11:45 AM. Pressure gauges ranges are 0 psig to 30 psig. The dispenser liners and transition sump were filled with water this morning at approximately 8:30 AM to the top of the entrance boots and was holding at the marked level at the time of this site visit. All fittings were soap tested with no apparent leakage. It appears that the secondary lines, dispenser liners, and transition sump are tight at this time. All primary piping remains at 50 psig on gauges with 0 to100 psig range. 05/15/2008 ¿ Jon Reeder on-site at 10:00 AM. Piping test boots in the transition sump were loose to permit interstitial monitoring. All shear valves were rigidly anchored. RELEASE DETECTION consists of the following: a. Teledata Tankmate SIR (EQ-466) w/triennial tank tightness testing for the single-wall tanks. b. Monthly visual inspections which includes interstitial monitoring of the double-wall piping at the transition sump. Inspections shall also include the dispensers, dispenser liners, and single-wall spill buckets. c. Mechanical line leak detectors and annual tightness testing for the single-wall lines. Overfill protection is drop tubes with flow shut-off. COMMENTS: Please note that the following documents are due within 60 days: 1. Limited Closure Summary Report for the piping refit. 2. Underground Storage Tank Installation and Removal Form for Certified Contractors. 3. A copy of the line tightness test results. 4. Incident Notification Form and investigation report with soil analytical results for the area surrounding dispenser #5/6.

There were no SRCO or NFA determinations found in OCULUS. Consequently, by virtue of Section 376.30716, F.S., and Rule 62-761.800(3)(b), F.A.C., no environmental closure assessment is required for the piping and dispenser liner refit / upgrade. However, since an incident was reported involving an automobile driving into the two store-side dispensers (4/22/2008) with a resulting fire in dispenser #5/6, an investigation should be completed to determine if a discharge occurred. At a minimum, this investigation should include OVA soil screening around dispenser area #5/6. This is a piping (partial) and dispenser liner removal / replacement only. Activity: Environ dispenser liners along with single-wall Ameron fiberglass piping was in the process of being removed and will be staged in a roll-off for future landfill disposal. All dispensers had been removed and the piping uncovered. No petroleum odors or staining were observed. The sand that had been used as a preventative berm around a County stormwater drain had been put into 55-gallon drums for proper disposal. Applied Science & Engineering was on-site.
