Description: SITE VISITS: Contractor: Guardian Fueling Technologies: PCC 056681 1212 North 39th Street, Suite 406 Tampa, FL 33605 Phone: 813-248-5707 6/20/2018 – Mark Trethewey on-site at 1:00 PM. Sump boots have been replaced with OPW Model DEB-4175 (EQ-684). Piping has also been replaced from garage to transition sump using OPW Pisces 1’ Flex pipe (also EQ-684). Double walled piping is installed within chase. 6/21/2018 – Mark Trethewey on-site at 8:00 AM. The hydrostatic test of the transition sump was started yesterday afternoon by adding water to a level past the highest entrance boot. The was no change in the water level at the time of this site visit. The new pipe was also pressure tested at 60 psi (primary) and 5 psi (secondary). Both sump and line appear to be tight at this time. INSTALLATION INSPECTION ONLY. Certain records and equipment associated with routine compliance inspections were not checked. COMMENTS: Due to the nature and age of the remaining underground “New-Oil” line, it is highly recommended that this line is replaced to avoid potential leakage and failure. Please note that the following documents are due as follows: 1. UST Installation and Removal Form for Certified Contractors is due within 30 days.

An environmental closure assessment is required for this transition sump entry boot replacement activity. Activities: Note: Per TXI/TIN of 8/29/2009; “There is a new-oil line that runs to the main dispenser (#1/2) island. This rubberized line runs through a PVC chase and is an extension of the dispensing unit inside the maintenance area. This line is not regulated as integral piping.” This line is being replaced from the garage outlet to the transition sump. Line was Weatherhead ¾” ColloCrimp #1130012, Manufacture Date of 7/18/1995. This is a sump boot removal and replacement only due to breaking concrete for the line, the sump and boots are regulated. ATC had been on site conducting field screening and sampling as needed. CLOSURE INSPECTION ONLY. Certain records and equipment associated with routine compliance inspections were not checked. The discharge currently has a score of 30 and is active. The eligibility is due to contamination from former underground petroleum storage tanks that were located near the existing dispenser island. An initial site assessment for this discharge was completed 2/8/1985. There were no SRCO or NFA determinations found in OCULUS. Consequently, by virtue of Section 376.30716, F.S., no environmental closure assessment is required for the piping monitoring sump replacement. This is a piping monitoring sump removal and replacement only. One (1) Environ piping monitoring sump had been removed and replaced with a Western Fiberglass unit (EQ-108). The sump was in good condition but was being replaced because the lid/cover was not water tight and, consequently, the sump would collect moisture during heavy rain events. A small amount of Ameron DW fiberglass piping (3¿/2¿, EQ-378, 3000/L) was also replaced/repaired. The primary lines had been pressurized with nitrogen at 50 psig and have held steady, within normal physical parameters, since 8:00 AM (initial: 50 psig, 9:00 AM: 51 psig). 08/29/2008 ¿ Jon Reeder on-site at 8:30 AM. The hydrostatic test of the piping monitoring sump was started at 7:00 AM by adding water to a level past the highest entrance boot. The was no change in the water level at the time of this site visit. 08/29/2008 ¿ Jon Reeder on-site at 1:45 PM. The secondary lines had been pressurized at 5 psig and have held steady, within normal physical parameters, since 12:15 PM. The water level was unchanged. Note: There is a new-oil line that runs to the main dispenser (#1/2) island. This rubberized line runs through a PVC chase and is an extension of the dispensing unit inside the maintenance area. This line is not regulated as integral piping. 09/09/2008 ¿ Jon Reeder on-site at 8:45 AM. Both sensors had been tested and were found to be operational. Piping test boots in the piping monitoring sump were loose to permit interstitial monitoring. The boots in the #1/2 dispenser liner were tight in order to only monitor the satellite lines. The alarm history for all sensors was printed to verify that an operability test had been conducted. There are no sensors in the piping transition sump or the interstice of tank #9. RELEASE DETECTION, as of 09/09/2008: a. Electronic interstitial monitoring of the piping monitoring sump, and master dispenser (#1/2) liner using Incon float sensors wired to an Incon TS 1001 controller (EQ-096): Probe 1 ¿ Tank #9 (facility #1) diesel ATG Probe 2 ¿ Tank #10 (facility #2) new/lube oil ATG Probe 3 ¿ Tank #11 (facility #3) used oil ATG Sensor 1 ¿ Dispenser #1/2 liner (DISP1/STD) Sensor 2 ¿ Piping monitoring sump (INST_SUMP) Aux. In. 1 and 2: Phone and FAX/Printer b. Monthly visual inspections which includes all components of the diesel tank and its interstitial leak gauge, diesel spill containment box, dispensers, dispenser liners, overfill system, and all components of the lube oil tank and its interstitial space. c. Mechanical line leak detector for tank #9. d. Overfill protection for the diesel tank (#9) is provided by an Incon overfill repeater alarm located on the South end of the tank. The backup system is an OPW 61F-STOP. e. Overfill protection for the lube oil tank #10) is provided by a Morrison clock gauge.

Underground Storage Tank Installation and Removal Form for Certified Contractors. NOTES: 1. The Incon overfill repeater alarm on the South end of the tank appeared to be out-of-order. The alarm test button was pushed which should have sounded a ¿test¿ alarm but it remained silent. It is recommended that you have this repaired. Meanwhile, the backup OPW 61F-STOP system will become primary. 2. The interstitial monitoring gauge for tank #9 indicates that the interstice has fluid. However, the Krueger liquid level gauge is not Florida approved for interstitial monitoring. This situation needs to be investigated and the proper gauge installed along with the removal of any liquids that may have accumulated in the interstice.
