Inspection covers the (2) 15000 gal. Emergency Generator Diesel, double walled USTs. FDEP Tank ID# 3R1 and 4R1. The Facility decided to remove the small portion of double walled steel piping that ran between the northeastern sump and the boiler room. Roughly 30 feet of the Primary metal pipe was removed. Reportedly all product was drained & collected prior to piping removal. The Secondary was left in place as a chase for the new flex piping being installed. The removed pipes were old, rusty, and pitted (took photos). However, no signs of a Discharge were observed. A site assessment is not required for this piping Closure. Received Email from Chris Collura at JMP Solutions stating they had gone to the site last night and found that one of the fuel supply pumps was wired so that it ran all the time. They corrected the wiring so that the supply pump only comes on when signaled by the day tank controller as it should. They were still not sure why the return pump did not function properly when tank was overfilled. They tested the system several times and the return pump cycled when the tank was filled. Chris recommends that the pump controller be replaced and a "high-high" level switch be installed as a safety factor to turn on the return pump in the event of an overfill. As it stands they only have a low level to turn the pump on & a high level to both turn the pump off and turn the return pump on. Chris is going to contact Simplex to build a controller to be integrates into the existing system. (see attachments for email chain from 6/10/2009 thru 7/08/2009) [Note: in speaking with Leslie Pedigo from SW District there was some doubt as to the size of the day tank. Citrus Memorial mentioned it might be a 600gal AST. The piping was not connected to either the tanks or the piping inside of the boiler room. It was laid in the trench with one end capped off and a pressure gauge on the other end. The primary was aired to 50lbs PSI and a small amount of pressure was released to test the gauges. All gauges held. The piping will be connected to the tank and the boiler room tomorrow and the secondary aired and soaped. The STP sump will be hydro tested after the penetrations are made. The sumps are existing sumps and will remain in place. See photos.

Subject: - Robert W. Deboard with ZHA General Consulting (407-422-7487) came to Citrus Co.EH office as a representative of Citrus Memorial Hospital. Dave Bauer & Sonya Ostensen were present during conference call with Laurel Culbreth. Subject: Open Violation for the unapproved equipment of the underground piping (Violation Chapter 62-761.850(2) FAC) The piping in question is two 1/2 inch copper lines (return & supply) that were installed with a PVC Secondary pipe back in 1994. These lines from from the UST's underground to a sump and into the Boiler Room. We discussed Citrus Memorial's options at this point: 1. replace piping with approved double walled piping 2. request a variance from the rule or 3. apply for an alternate procedure. The issue is that the hospital is to undergo an expansion which will entail removing the UST's (that run the boiler and generators) and lines and install new tanks to run generators in a different area. According to Mr. Deboard this is not going to start for another 2 years at this point in time. Thus Citrus Memorial does not want to spend money replacing the piping when it is going to be removed during the planned expansion. We discussed the Alternate Procedure option at length. Laurel said the purpose would be to increase release detection on the piping and that a time line would most likely be needed. We discussed ideas of increasing the frequency of release detection on the pipe interstice by visually or electronically inspecting the sump once a week instead of once every 35 days, doing a breach of integrity test on the secondary once a year, & giving the Alternate Procedure a 2 yr deadline at which point the piping would need to be either removed or replaced with approved piping. Laurel also suggested that they get with a PCC contractor to see if they had other ideas to increase release detection. Mr. Deboard said that was a reasonable solution and that Citrus Memorial will probably be likely to do the Alternate Procedure. We discussed the Form that needs to be filled out ( I showed Mr. Deboard the form on the website, printed him a copy, and also email the website link to his email account), DEP has 60 days to either approve or deny the application. We determined that the 180 days for the open minor piping violation has passed since the orginal issue was discovered and written up on 10/16/2008 (TCI by D.B.). Laurel said to give Citrus Memorial Hospital 30 days from today since we have down graded the violation from a Snc to a minor (originally wrote the wrong violation on TCI). Thus Citrus Memorial Hospital needs to apply for an Alternate Procedure and send to Bill Burnes (DEP Tallahassee) no later than 5/23/2009. After we ended the call with Laurel, Dave and I went over the other 4 open violations. We told Mr. Deboard that we have received nothing from Citrus Memorial to resolve these violations. He already had a copy of our inspection report and made notes, he said Citrus Memorial is making every effort to comply. Mr. Deboard said they would be in touch. I told him to feel free to call me or Dave and we would help them fill out the Alternate Procedure Form.

I explained that on 11/24/2008 we received a final ruling from Tallahassee & OGC that the entire systems of the UST's that feed both the boiler room and the emergency generators are regulated. Which means all of the piping is regulated and thus the open violations stand. (regarding the two 15,000gal UST's in front of hospital). Ultimately this means that the double walled steel piping run from the sump to the boiler room needs some type of cathodic protection and that the copper piping with pvc chase needs a secondary pipe that meets code. I explained all this. Joe requested the email from OGC ruling this. I forwarded the email from OGC with the final ruling as well as Dave Bauer's original email posing the question with a site diagram and sump pictures attached. Joe asked about the 30 day deadline to resolve these issues. I explained that since we just got the ruling from tallahassee on 11/24/2008 that the time line will start over. And that as long as he has a plan or contract by the end of this next 30 days DEP will not pursue enforcement. I told Joe to keep communicating with me on how they plan to resolve the piping issues. They plan to put an extension on the sump so that the cover is out of the ground as the area collects the water from the roof and is normally wet after a rain. Both 15,000gal tanks are connected to the boiler room with single walled copper piping in a PVC chase and used for the boiler and/or emergency generator. There is a supply and return line in each sump. The pump for these tanks is inside the boiler room area so piping goes from the tank to the pump and then to the generators via steel piping in a steel secondary. The steel piping goes from the boiler room to a transition sump in front of the tanks on sidewalk. The piping inside this sump transitions from the steel piping to a double walled green Environ flex pipe for both the supply and return. See photos. There is a question whether the copper piping in the PCV chase is a legal double walled system. Both the primary and secondary were pressure tested when installed. There is also a question if the steel piping in the steel chase(need to verify it is steel and not fiberglass) needs to be cathodically protected.

Description: Fills Marked per API 1637: All three are painted for diesel Overfill Prevention: Flow shutoff on the 8,000gal tank by the school and ball check valve on the two 15,000gal tanks located at the front of the hospital by the main entrance. Spill Bucket Condition: ( 3 total) The two front buckets are clean and dry and constructed of metal. The rear bucket had 1-2 inches of liquid inside and it is a bellows type bucket in good condition with no cracks or rips noted. Piping / STP Sump Condition: ( 7 total) 8,000gal tank ¿ STP sump has small amount of water inside but not over sensor or piping. It is clean with two STPs and two MLLDs installed. The transition sump has 1-2 inches of water inside. Alarm is in water. Water is below piping invert and needs to be pumped out because of the alarm. Piping: Copper on tank 1 and 2 and dw fiberglass (2 inch in 3 inch) on tank 3. The piping from the boiler room pump to a transition sump is metal piping inside a metal chase. The piping from that transition sump to the another sump is green Environ flex piping and from there to the transition sump near the generators is also green Environ flex that transitions to galvanized steel that is aboveground. That piping is painted and no cracks or leaks were seen. All piping has a supply and return line. D.W. Description: TCI was performed on 10/11/06 Toughbook had virus thus the delay of data entry into FIRST Placard: 2006/2007 at facility RDRL: On file at facility but not required to have. CFR: On file at facility, Ins.-Commerce & Industry 7/30/06-7/30/07 Release Detetion is not required for Emergency Generator UST's and the AST is only 550gal and therefore not regulated by DEP. However, monthly visuals of sumps, fills, & sensors are conducted and recorded each month. Looked at records from 1/2006 thru 10/2006. There are 7 total liquid sensors and 2 leak detectors on site. Each were tested by Gaurdian Fueling Technologies on 8/31/06. All passed. Next operability tests are due 8/31/07. Fills are marked per api 1637 - diesel Overfill protection is flapper valves and ball check valves. Spill buckets: Diesel SB on West side is full of liquid-over fill cap, on East side the diesel SB on right side (facing hospital) has about 5 inches of product, SB on left side is dry with debris that needs to be cleaned out. Sumps: Transitional sump on West side has 1 inch of liquid, Piping is DW and looks rusted inside sump. (Took Photo) The two Diesel sumps on West side are equipped with STP's, MLLD's (red jacket), & DW fiberglass red piping by Ameron (Took Photo of slightly rusted diesel sumps). Transitional piping sump near the day tank has .5 inch of liquid - appears to be water. Sump to the Diesel UST on East side has 4 to 5 inches of liquid (no sheen). This piping sump is registered to L4 (monitored by a Veedor Root) which is in alarm. Pulled tape on Liquid sensors for East side - see attachments. Double walled pipe interstice was open in both sumps on West side. Liquid sensors were not installed properly in the sumps on the East side - this was corrected at the time of inspection. There are four liquid sensors on the East side; one in each sump and one in each tank interstice. There are three liquid sensors on the West side; one in each sump and one in the tank interstice. L4 has been in Alarm since August indicating liquid in the sump. The three LS on the West side are monitored thru EECO System 1500 series by EMCO Electronics. Could not get a tape from this system.
