Please supply resolving documentation as soon as possible. As noted in the report: The PUL & DSL fill ports should be locked to ensure tanks are not filled. *The PUL double-walled spill bucket failed vacuum testing on 6-2-20. The violation will be closed at this time based on the following: PUL fuel has not been sold at this tank for around four years. A review of monthly inspections indicated the PUL tank has been empty. The PUL tank compartment was empty at the time of the 3-3-22 inspection. The PUL fuel supply is not connected within the dispenser sumps. **Moving forward, if PUL fuel is sold, the following requirements must be met: Please arrange for repairs to be made to the PUL spill bucket. To check the integrity of the spill bucket, please perform a hydro-test on the primary buckets. If the hydro passes, please install a new primary spill bucket insert. Post-installation testing should be supplied for review. If the hydro fails, please replace the whole spill bucket. Please submit hydro-testing results to Brittany.Finney@ocfl.net. In the event that the whole spill bucket needs to be replaced, soil sampling will be required. Also, a certified contractors form is required whenever concrete is broken. The following records should be onsite, or supplied electronically within 5 days of the inspection: the current registration placard should be posted onsite tank insurance policy from 6/2/21 - present Testing needed: 2021 release detection testing, line leak detector testing, overfill protection equipment testing, & integrity testing of the PUL spill bucket OPEN VIOLATION FROM 2020 INSPECTION: Operator A/B & C training certificates were not supplied; Facilities owners are required to complete the A/B training & store managers/clerks are required to complete the C training. Description: On site for routine compliance inspection. Placard was not posted on site. Environmental book on site with two years of documentation and testing results. Veeder Root appeared to be working properly. There was a low fuel alarm on the veeder root. Liquid status was showing normal. Premium sump did have fluid that was below the piping penetrations. Premium and regular sumps had some ethanol corrosion that will need maintaining soon. Probes were properly placed. Piping was open to the sumps. Dispenser pans were dry. Shear valves secure, hoses and nozzles acceptable. Fill ports were painted. Spill buckets were stainless steel and in good condition. They were dry at time of inspection. BOI is current. Registration appears up to date. No EPA training available. Laptop not available for signature. Report emailed to Omi Mirchandani.

Per FS 376.3077, "It is unlawful for any owner, operator, or supplier to pump or otherwise deposit any motor fuel into a tank required to be registered under section 376.303 FS, unless proof of valid registration is displayed on such tank itself or the dispensing or measuring device connected thereto or, where appropriate, in the office or kiosk of the facility where the tank is located." Penalties can be assessed to the facility owner for failure to properly register tank systems. Note that penalties can be assessed to the supplier that delivers motor fuel to an unregistered facility. Check the details on the placard. Do they agree with the location you are inspecting? Owners of multiple facilities may switch placards by mistake. Placards are not always issued or received in a timely manner at the change of year period (June and July). Provide the DEP Tallahassee phone number to the facility so they may resolve problems (850) 245-8850; (850) 245-8840. Check STCM in the registration/placard section for the date that the payment was made and the date that the placard was issued. The location of the placard in plain view can be subjective in interpretation: a. For vehicular fuel facilities, plain view is meant to allow the transport delivery driver to view the placard prior to delivering fuel. b. A legal definition is an exception to the general requirement of a valid search warrant to legitimize a search or seizure; a search implies prying into hidden places, and it is not a search to observe what is open to view.

Examine the annual function test data - a pass alone is not sufficient. The tester must demonstrate 3.0 gph discharge detection for the mechanical (flow restrictor). Note that the electronic (auto shut off) line leak detector does not need an annual test, but must still be able to detect a 0.2 gph leak after it has been tripped at 150% line operating pressure. Replacements - unit must be tested out of the box - function must be proven. 3.0 gph = 185.4 ml/min (To convert milliliters/minute to gallons/hour, multiple the milliliters by 0.0162162; or, 1 gph equals 61.8 ml/min). Values greater than 186 ml/min fail. Values less than 185 ml/min mean that the line leak detector will activate at a more restrictive rate.

Examine the annual function test data - a pass alone is not sufficient. The tester must demonstrate 3.0 gph discharge detection for the mechanical (flow restrictor). Note that the electronic (auto shut off) line leak detector does not need an annual test, but must still be able to detect a 0.2 gph leak after it has been tripped at 150% line operating pressure. Replacements - unit must be tested out of the box - function must be proven. 3.0 gph = 185.4 ml/min (To convert milliliters/minute to gallons/hour, multiple the milliliters by 0.0162162; or, 1 gph equals 61.8 ml/min). The vacuum readings were as follows: Manifest-25 hg; Arrival- 27 hg; In place-26.5 hg. The UST was a 22,000 gallon- MPC Plasteel- Elutron Compartmented Tank (EQ#333). The compartmentation was 12,000, 6,000, and 4,000 gallons. The deadmen were placed properly and the tie-downs were installed correctly. Description: 2010.03.16-Closure-I arrived on site at 8:15 am. The tanks were being removed by Petroleum Technicians Inc.(PCC#0566994). Tom Roy- Foreman on site. The tanks were cleaned out by Petrotech-Southeast Environmental services. The soil sampling was conducted by Bill Good - G&S Environmental services.There were some high soil sample readings around the fills. The USTs removed were in good condition showing no rust, corrosion or holes. In all, there were three 8,000 gallon and one 550 gallon (Buffhide Buffalo) fiberglass clad steel tanks removed.The excavation appeared to be clean- no strong odor or free product was present. The water table was below 20 ft.
