| Description: | Facility Name: Champs Sports Installation Inspection Field Notes 08/19/2010 ¿ Mark Trethewey on-site, contacted by Facility due to supplier refusal to refill. Please note that this inspection is being completed after-the-fact. It is estimated that the tank system was installed in October of 2006. Tank is fueled by: Bradenton Fuel Oil, Inc. 6116 21st St., E. Bradenton, FL 34203 (941) 741-3900 Contractor: Power Secure, Inc. 1609 Heritage Commerce Ct. Wake Forest, NC 27587 (919) 556-3056 The Caterpillar GenSet with a 1,000 gallon belly tank was on-site and in-service. The tank was manufactured by: JRS, Inc (EQ-497) 186 NW 68th Avenue Ocala, FL 34482 Phone: (352) 351-2100 Model #JRS-1000 Tank is equipped with a Morrison Brothers spill bucket (EQ-345), Krueger sight gauge w/alarm (EQ pending) for overfill protection, and an electronic interstitial float sensor wired to NexGear Switching Unit for relay of alarm status to Power Secure in North Carolina. Inspections of the system are performed on the First and Third Wednesday of the month, inspection sheets were provided to include condition of all tank components. RDRL was also provided at inspection. Passing Annual Operability Test of the interstitial sensor was not provided at inspection, passing test performed on 9/1/2010 was supplied after inspection. Current Proof of Financial responsibility was not provided at inspection. FR from 8/20/2010 to 8/20/2011 was supplied after inspection. STRF was signed for submittal to FLDEP. (Note: Facility ID received on 8/27/2010). Historical note: This Facility was previously issued a registration in May 1989 as a result of a contamination assessment performed December 1988. A UST was suspected but determined to have been removed sometime prior to assessment. RELEASE DETECTION, as of 8/19/2010: a. Monthly visual inspections which includes electronic interstitial monitoring of the double-walled tank. Inspections shall also include the tank exterior, spill containment, fill gauge, generator interior, and surrounding area. b. Annual operability test of the tank interstitial sensor. Please note the following: 1. Monthly visual inspections must include remote verification of proper operation of the tank interstitial sensor. 2. The tank interstitial sensor must be physically operability tested annually. ENFORCEMENT ACTION TO BE INITIATED.
