One OPW DS1928 dispenser liner (EQ #318) and Amaron / NOC LCX piping (EQ #291) have been installed. ICON (EQ-896) sump boots have also been installed. NOTE: Approximately 2’ of Diesel piping and clamshells were replaced along with approximately 6’ of Regular piping and clamshells. Remainder of piping to transition sump remains untouched. The primary lines had been pressurized since Monday morning at 55 psig and have held steady, within normal physical parameters. (Regular – set 55 / now 55; Diesel – set 55 / now 55. The secondary lines had been pressurized since Monday morning at 5 psig, the Regular has held steady, within normal physical parameters. (Regular – set 5 / now 5). The Diesel has not held pressure, investigation to locate leak has started. A hydro test was also performed on the dispenser liner. Water was introduced yesterday and continues to hold at the mark. 3/21/2024 – Mark Trethewey on-site at 1:40PM. A cut was discovered in the outer wall approximately 150’ East from the transition sump. Piping was repaired. The Diesel secondary line had been pressurized since Tuesday afternoon at 5 psig, and has held steady, within normal physical parameters. (set 5 / now 5). All line and sumps appear to be tight at this time. Dockside shear valves are secure. CAO issued with this inspection is due to open violations from TCI, not associated with Install. INSTALLATION INSPECTION ONLY. Certain records and equipment associated with routine compliance inspections were not checked. Please note that the following documents are due as follows: 1. Copies of the line and liner tests are due within 30 days.

Foam Installer: Southern Foam Insulation, Inc. 2593 Clark St., #108 Apopka, FL 32703 (407) 293-8773 Environmental Cleaning: Aqua Clean Environmental 3210 Whitton road Lakeland, FL 33811 (863) 644-0665 Contamination Status: This site is not in a program. It has no score and is not in active cleanup. There were no determinations found in OCULUS. Consequently, by virtue of Section 62-761.800(3)(a)., a complete environmental closure assessment is required. Activities: As required by Condition A of EQ-672, a flotation calculation was provided by Bert Thomas, registered Architect #10098. The calculation indicates a safety factor of 1.81 (downward forces/upward forces) to prevent float-out. The tanks and all piping had been cleaned using the Butterworth method and triple rinsed by Aqua Clean Environmental, Lakeland, on 12/17/2009, prior to closure-in-place. The piping was also cleaned and blown back to the tanks. The vent piping will be cut near the elbow where it exits the ground and caped at both ends. All other piping was capped. Two (2) 10,000 gallon single-wall, fiberglass, Owens-Corning underground petroleum storage tanks were closed-in-place with the application of inert foam from cfiFoam, Inc. (EQ-672), applied by Southern Foam Insulation, Inc. The two dispensers were replaced and re-piped with the installation of an AST in 11/2009. The tank owner provided the following documents during the closure inspection: 1. An updated Storage Tank Registration Form. 2. Buoyancy calculations conducted per PEI practice #100-87. 3. Manifest for the tank cleaning process. FINANCIAL RESPONSIBILITY: Financial Responsibility has not been provided for the UST systems. CLOSURE INSPECTION ONLY. Certain records and equipment associated with routine compliance inspections were not checked. COMMENTS: Please note that the following documents are due within 60 days: 1. Contractor: Guardian Fueling 1212 N. 39th St. Tampa, FL 33605 Office: (813) 248-5707 Cell: (941) 812-5671, Chuck Innes Cell: (813) 312-4364, Troy Activities: One (1), 8,000 gallon ConVault, UL-N20208, double-wall steel / concrete aboveground petroleum storage tank (EQ-201) had been delivered on-site and set on a concrete pad. This system is FDEP approved as a complete unit and requires no testing upon delivery. The tank has two compartments: 5,000 gallons for diesel and 3,000 gallons for mid-grade gasoline. The storage tank system is comprised of the following: factory-constructed spill containment, two Morrison clock gauges (EQ-411) for overfill protection, and a Krueger Type K leak gauge (EQ-675) for interstitial monitoring. The tank will also be equipped with ASCO solenoid valves located on the take-off piping. Black iron aboveground piping for the Diesel product and galvanized aboveground piping for the Plus product will run into an OPW Pisces piping transition sump (EQ-368) where it will connect to the existing Ameron 3000/MCX coaxial double-wall piping (EQ-562) that runs under the pier to a dispenser for fueling boats and also to a nearby land-based dispenser which is over an Environ dispenser liner (EQ-439). The new piping from the new transition sump to the land-side dispenser will be aboveground and under the pier. It will be Ameron double-wall, replacing the existing Ameron single-wall, with OPW isolation tubes (EQ-369) protecting the flex piping underneath the existing dispenser liner which is over now water. The contractor states that the isolation tubes protecting the flex pipes are attached over the end of the double-wall piping and forms a continuous interstice from the bottom of the dispenser sump to the transition sump. 12/10/2009 ¿ Jon Reeder on-site at 1:00 PM. Work is in progress installing the piping from the new transition sump to the land-based dispenser. All lines going from the transition sump to the tank systems have been cleaned and capped at the old transition sump end. The old transition sump has been removed and sent to landfill for disposal. 12/11/2009 ¿ Jon Reeder on-site at 1:30 PM. The gasoline and diesel primary lines had been pressurized to 60 psig at 12:30 PM on 12/11/2009 and have held steady within normal physical parameters. (Plus ¿ set 60 / now 61; Diesel ¿ set 60 / now 61) The contractor soap-tested all fittings with no problems noted. It appears that the primary lines are tight at this time. This pressure test of the primary covered both the new and existing DW piping and the SW black-iron piping leading from the transition sump to the tanks. 12/14/2009 ¿ Jon Reeder on-site at 10:00 AM. All clam-shell fittings had been installed. The secondary lines were pressurized at 3 psig this morning and have held steady within normal physical parameters. (Plus ¿ set 3 / now 3; Diesel ¿ set 3 / now 3) The secondary on the existing Diesel line from the transition sum to the dispenser on the pier failed the pressure test due to a cracked entrance boot at the dispenser liner. This will be repaired in the near future. The contractor soap-tested all fittings with no problems noted. It appears that the secondary lines are tight at this time. This pressure test of the secondary only covered the new piping and existing gasoline line. Both primary lines were still holding air at 60 psig. Water had been added this morning to the transition sump and land-side dispenser liner up to a point above the highest entrance boot. The levels were unchanged at the time of this inspection. It appears that all secondary containment sumps are tight at this time.

Tank interstitial monitoring using a Krueger Type K leak gauge. The status of the interstitial leak detector should be noted on the monthly visual form. b. Contractor: Guardian Fueling 1212 N. 39th St. Tampa, FL 33605 Office: (813) 248-5707 Cell: (941) 812-5671, Chuck Innes Cell: (813) 312-4364, Troy Activities: One (1), 8,000 gallon ConVault, UL-N20208, double-wall steel / concrete aboveground petroleum storage tank (EQ-201) had been delivered on-site and set on a concrete pad. This system is FDEP approved as a complete unit and requires no testing upon delivery. The tank has two compartments: 5,000 gallons for diesel and 3,000 gallons for mid-grade gasoline. The storage tank system is comprised of the following: factory-constructed spill containment, two Morrison clock gauges (EQ-411) for overfill protection, and a Krueger Type K leak gauge (EQ-675) for interstitial monitoring. The tank will also be equipped with ASCO solenoid valves located on the take-off piping. Black iron aboveground piping for the Diesel product and galvanized aboveground piping for the Plus product will run into an OPW Pisces piping transition sump (EQ-368) where it will connect to the existing Ameron 3000/MCX coaxial double-wall piping (EQ-562) that runs under the pier to a dispenser for fueling boats and also to a nearby land-based dispenser which is over an Environ dispenser liner (EQ-439). The new piping from the new transition sump to the land-side dispenser will be aboveground and under the pier. It will be Ameron double-wall, replacing the existing Ameron single-wall, with OPW isolation tubes (EQ-369) protecting the flex piping underneath the existing dispenser liner which is over now water. The contractor states that the isolation tubes protecting the flex pipes are attached over the end of the double-wall piping and forms a continuous interstice from the bottom of the dispenser sump to the transition sump. 12/10/2009 ¿ Jon Reeder on-site at 1:00 PM. Work is in progress installing the piping from the new transition sump to the land-based dispenser. All lines going from the transition sump to the tank systems have been cleaned and capped at the old transition sump end. The old transition sump has been removed and sent to landfill for disposal. 12/11/2009 ¿ Jon Reeder on-site at 1:30 PM. The gasoline and diesel primary lines had been pressurized to 60 psig at 12:30 PM on 12/11/2009 and have held steady within normal physical parameters. (Plus ¿ set 60 / now 61; Diesel ¿ set 60 / now 61) The contractor soap-tested all fittings with no problems noted. It appears that the primary lines are tight at this time. This pressure test of the primary covered both the new and existing DW piping and the SW black-iron piping leading from the transition sump to the tanks. 12/14/2009 ¿ Jon Reeder on-site at 10:00 AM. All clam-shell fittings had been installed. The secondary lines were pressurized at 3 psig this morning and have held steady within normal physical parameters. (Plus ¿ set 3 / now 3; Diesel ¿ set 3 / now 3) The secondary on the existing Diesel line from the transition sum to the dispenser on the pier failed the pressure test due to a cracked entrance boot at the dispenser liner. This will be repaired in the near future. The contractor soap-tested all fittings with no problems noted. It appears that the secondary lines are tight at this time. This pressure test of the secondary only covered the new piping and existing gasoline line. Both primary lines were still holding air at 60 psig. Water had been added this morning to the transition sump and land-side dispenser liner up to a point above the highest entrance boot. The levels were unchanged at the time of this inspection. It appears that all secondary containment sumps are tight at this time.
