Subject: Spoke with Will Irby the representative of the facility regarding the status of the tank closure. Mr. Irby informed that steps were being taken to close the tank this year. Mr. Irby was informed that a since the tank primary had failed and no way to test the secondary a closure assessment with sampling would be required. The tank is currently empty and has been cleaned. The tank has been registered as out of service. Mr. Irby was informed that even though the tank is out of service, storage tank insurance and the annual placard is still required. A recommendation has been made to close the tank in place by foaming.

Description: The vacuum gauge has not been tested in over a year. Have the gauge tested and send the report to the local program offices. Comments: One 1 compartmented DW UST with 2 dispensers; suction system. All monitoring is visual; the UST is sticked for product level and there is a vacuum on the interstice. Overfill prevention is by flapper valve. The sumps and spill buckets were dry and appeared tight; OPW piping. White dispenser liners were dry, hoses and nozzles are good. Little if any vacuum on the interstice. There is a hose over the water to a hose reel on the dock. This should be re-arranged. Have the hose reel on shore. The area is clean. The registration placard was not on site; print and post the placard. The RDRL was in the log. No UST training certificates; go online to the address listed on this report to earn a certificate. Discharge Information: No discharges This report will be mailed via USPS. A non compliance letter citing the violations accompanies this inspection report; please find it attached. Please provide our office with documentation confirming the resolution of the cited violations within thirty days of receipt of this inspection report and attached non compliance letter. Description: This is the spill bucket, piping sump, product piping and dispenser sumps closure by removal report for this facility which occurred the week of 12/27/2010. The removal contractor is Ocala Pump and Tank, of Ocala, Florida (PCC# 050768). The Enviornmental Consultant is Land Environmental, of Newbwerry, Florida, represented by Mr. Kurt Howell. Removed, successfully, were the following equipment: A fillport spill bucket (singlewall steel by OPW); product piping (doublewall fiberglass A.O.Smith redthread); two dispenser containments( plastic by Containment Technologies, black in color) and a piping sump (plastic by Containment Technologies, black in color). All the removed equipment appeared to be in fairly good shape, with no apparent holes or degradation related punctures and no breakage. It is to be noted that a small portion of the former piping was drained and capped off in the ground (near the two dispenser areas). Most of the piping was removed. All removed equipment was transported off-site for proper disposal as salvage. A closure assessment report will be provided to our office. Photos of this closure event is attached to this report.

Description: This is the installation inspection report for the installation of a new spill bucket, a sump containment, two dispenser sumps and piping, that occurred on 12-29-2010. The newly installed piping was OPW Pisces double wall synthetic (EQ- 649). The spill bucket is EMCO Wheaton, doublewall (EQ-695). The piping sump is OPW fiberglass (EQ-318). The two dispenser sumps are OPW flexworks (EQ-318). The contractor performing the installation is Ocala Pump & Tank of Ocala, Florida (PSS#PCC 050768). December 29, 2010 The primary piping was pressurized to 50 psi and the secondary piping was pressurized to 5 psi, for both regular gasoline lines. The leak testing was performed with no leaks found anywhere. The pressure gauges were bled down to demonstrate the presence of pressure and that the gauges were operational. No problems were noted. February 2,2011. All new system start-up tests were performed by Ocala Pump & Tank. All tests passed.
