| Description: | On Site 7/17/2024 I, Calvin Williams, Southeast District of the Department of Environmental Protection, met with Aaron McKinney a representative of Integral Company and LeAndra Monroe. A VISUAL inspection showed that the temporarily closed facility had some debris on the tank pad. The tank had fluid in it. It needs to be pumped out. The spill bucket was intact, but the spill bucket had some water in it and the lid was missing. Vent pipe was present. RELEASE DETECTION through visual inspections were not available. The PIPING sump was not accessible due to dirt covering the inner lid. Building construction is happening near the tank pad. The external lid was missing. The STP sump had a piece of board as a lid. RECORD of CFR is dated from 2/1/2024 to 1/31/2025. On the Certificate of Financial Responsibility, the address is correct, but the name of the facility needs to be updated. No operator licenses were not present. The aforementioned violations were cited in 2021. The placard was dated from 7/15/2024 to 6/30/2025. I inspected a 600-gallon underground storage tank. The tank was used for an emergency generator. Currently the tank is registered out of service and the building is vacant and uninhabitable. The out of service status started on 8/25/2015. Please remove all regulated substances from the tank so that no more than one inch remains. A VISUAL inspection showed that the tank pad appeared to be in good condition with no cracks. Construction debris was on the pad, but it was removed. The camera must have malfunctioned and did not save the other pictures. Therefore, one picture is available. Spill bucket for the fill port was not properly color coded because the lid was missing. The spill bucket was filled with water. Vent pipe was present. PIPING was not inspected due to the level of water in the sump. Also, the lid of the piping sump was missing RECORD of CFR is listed in the county’s local bond. The name is registered as the previous potential tenants. It is dated from 2/1/2021 to 1/31/2022. I inspected a 600-gallon underground storage tank. It is a tank used for an emergency generator. Note: The tank has been registered out of service since 8/25/2015, because the facility is closed. The new owners of the property are UHWG. Please submit a new registration form to reflect the new owners. A VISUAL inspection showed that the tank pad appeared to be in good condition. Please verify if the tank has water in it or diesel, because Rule 761.801(1) requires removal of all regulated substances so that no more than one inch in depth regulated substances remains in the storage tank; and facility owners and operators of out-of-service storage tank systems shall monitor the interstice and the liquid level in the storage tank annually but not to exceed 12 months, unless the tank system contains no regulated substances. Spill bucket for the fill port was properly color coded. The spill bucket was intact but it had some water in it. OVERFILL PROTECTION was provided by a flapper valve. Vent pipe was present. RELEASE DETECTION is provided by an electronic system that is out of service due to no electrical power from the closed building. PIPING appeared to be in good condition, but the sump had water in it due to the rain. RECORD of CFR is dated from 2/1/2018 to 1/31/2019. Placard was dated from 5/17/2018 to 6/30/2018. I met with Mr. Javier Figueroa with Jackson Memorial Hospital The inspection was conducted as a follow up to the previous inspections in which the registration does not appear to have been updated. The single walled tank was removed according to the March 3, 2010 Closure Inspection. A new 600 gallon double walled fiberglass tank was installed on April 1, 2010. As of the date of the inspection FIRST reflected only one out of service fiberglass UST. I inspected the accessible storage tank components and found everything to be in good shape. There was no excessive weathering on any of the components. I filled out a registration form and Mr. Figueroa signed it. The form was submitted to Tallahassee on 8-21-2015 via email. According to JMH personnel, the facility will be handed over to the City of Opa Locka. I gave Mr. Figueroa a registration form to use once the property is transferred. | ||||||||||

| Description: | Teresa Rahrig (TR) from the SED Office Storage Tank Section first spoke with Jose Camero, (305-585-1302/4179),Jackson Memorial who referrred me to Errol Drummond (954-243-5461) to conduct an inspection of the closed storage tank facility at 14701 NW 27th Ave. N Maternity Bldg. at 12:00 PM on April 11, 2014. Although Mr. Drummond was not there, he arranged for Jean Simon, a maintenance worker and security to assist TR to gain access to the compound for an inspection. Note that the registration records reflect an Out of Compliance for failure to upgrade, an Out of Service 550 gallon single walled tank. The database was never updated with a Storage Tank Registration Form for the new 600 gallon double walled tank installed in April 2010, replacing the previous tank. I located a registration form, completed by the contractor, that should be reviewed and updated and forwarded to Tallahassee Registration Section. Note that the status Code U for an In Service tank should be changed to the Out of Service Status Code T. The tank is associated with an emergency generator inside the building. Registration should be updated to include the current registration codes for this facility. The Responsible Party Information is currently incorrect and lists David Krenson w/incorrect phone number as the facility contact to set up inspections, Francisco M. Fuentes as the Account Owner and Jerry Phaff as the Tank Owner. Please correct these contacts and phone numbers and add the Property Owner and Facility Owner also to the State Registration Form, submitted with this inspection. Note that the piping sump was inaccessible, as there are hex nuts holding down the sump cover that could not be removed by the maintenance worker. The spill bucket was full of water and had to be bailed, to open the fill port. A gauge stick was emersed down the fill of the tank, and indicated less that 1 inch of product, which is the definition of an Out of Service tank. Note that every 6 months, inspections are recommended, but not required, (Emergency Generator USTs are exempt) for examination of the sumps for piping external releases or operating conditions of the tank and piping configuration. Note that a wet, corrosive environment may cause corrosion and future equipment failures. Note that the external spill bucket which is secondarily contained, is compromised as evidenced by the gauge reading. See red indicator in the photos. Water may have entered the interstitial space of the bucket. The emergency vent is intact and in good condition. Although monitor well bailing every 6 months may be a county code requirement, there was no evidence that the wells are bailed routinely. The covers are properly color coded. No State paperwork or records were at the facility, including a Certificate of Financial Responsibility State Form 62-761.900-3, or current registration placard. A State CFR form will be forwarded with this inspection. The underground piping emerged from the ground secondarily contained and to the generator room. A TMS 2000 Pneumercator panel is in the building, but power is out in the building and is not being used. Followup: Please provide a current, completed State of Florida CFR form, as the facility is listed in the County Bond List for Storage Tank Insurance, as required by State Rule. Additionally, Update the State Registration Form and resubmit, as noted above. It is recommended, although not required to visually inspect and maintain sumps dry every 6 months while the tanks are in the Out of Service status and to remove any water/contaminants from the spill bucket. This report will be electronically issued to Jose Camero of Jackson Memorial. See attached CFR, Completed registration form from the 2010 contractor, blank registration form. Retained only one outstanding violation from 2010 which is recordkeeping violation for not maintaining State CFR form. | ||||||||||

Owners of multiple facilities may switch placards by mistake. Placards are not always issued or received in a timely manner at the change of year period (June and July). Check STCM in the registration/placard section for the date that the payment was made and the date that the placard was issued. The location of the placard in plain view can be subjective in interpretation: a. One 550 gallon single-walled fiberglass tank was properly removed. Tank excavation size was 10 X 6 X 8. All excavated soils to be used as backfill in conjunction with clean pearock to fill tank excavation. 550 gallon single-walled tank was cleaned out by PERMA FIX approximately 200 gallons total out of tank of pressure clean water was removed from tanks. Tank was cleaned before removal from excavation. Tank was observed to be intact and in good condition during my inspection. All holes noted were placed there by backhoe after removal from excavation. All associated piping has also been removed and disposed of. Tank will be crushed and taken to landfill for disposal. Manifest for tank clean out by PERMA-FIX. There was a mix up of inspection sites, so Mr. Vaga was not expecting an inspection. He was very cooperative under the circumstances. This facility has been downsized and is used as a Health Care Center. Inspection of tanks: Fill boxes correctly color coded Spill containment clean and dry Sumps were bolted down. Not inspected. This an emergency generator tank. Release detection is not required. Monitoring wells are being sampled using a bailer. Dispensers: No dispensers. This is a suction system Inspection of Piping: Mr. Vaga says the piping was upgraded to double wall piping. Inspection of Monitoring System: This is an underground emergency generator tank. Inspection of Records: Registration fees paid. Placard Posted. Release Detection Response Level (RDRL) posted and up to date. Monthly Monitoring Records Maintained for 2 years. Per FS 376.3077, "It is unlawful for any owner, operator, or supplier to pump or otherwise deposit any motor fuel into a tank required to be registered under section 376.303 FS, unless proof of valid registration is displayed on such tank itself or the dispensing or measuring device connected thereto or, where appropriate, in the office or kiosk of the facility where the tank is located." Penalties can be assessed to the facility owner for failure to properly register tank systems. Note that penalties can be assessed to the supplier that delivers motor fuel to an unregistered facility. Check the details on the placard. Do they agree with the location you are inspecting? Owners of multiple facilities may switch placards by mistake. Placards are not always issued or received in a timely manner at the change of year period (June and July). Provide the DEP Tallahassee phone number to the facility so they may resolve problems (850) 245-8850; (850) 245-8840. Check STCM in the registration/placard section for the date that the payment was made and the date that the placard was issued. The location of the placard in plain view can be subjective in interpretation: a. For vehicular fuel facilities, plain view is meant to allow the transport delivery driver to view the placard prior to delivering fuel. b. A legal definition is an exception to the general requirement of a valid search warrant to legitimize a search or seizure; a search implies prying into hidden places, and it is not a search to observe what is open to view. | ||||||||||
