A proposal dated March 10, 2010, from Norris & Samon to attempt repairs to the piping entry boots was provided by Braden Thurber during my site visit to the facility on March 19, 2010. He stated that the proposal was sent to corporate and was pending approval and that I would be notified when the work proceeded and could witness a hydrostatic test. Since that visit, I have received no notification from the facility through the date of this inspection. During record review at the beginning of this inspection, Braden Thurber stated that they had finished the repairs and performed a succesful hydrostatic test of the sump since my visit in March. During the physical inspection, an employee stated that the attempted repairs were actually carried out by a Loggerhead employee, Wes Nichols, by applying a product called 3M Marine/Adhesive Sealant 5200 to the piping penetration boots and donuts. It does not appear that this product is approved for this use by the manufacturer of the penetration seals, Environ/OPW. The facility mechanic that uses the product for boat repairs stated that the material is not compatible with fuel, and can be cleaned-up with mineral oil. I advised Braden that the material was very unlikely to be approved by the manufacturer of the boots and it would have to be removed, and then they must complete approved repairs or replacement of the sump. OPW dispenser liners: landside dispenser dry, piping donut pushed-out of penetration seal for the OPW piping, sensor raised-up above piping level; North dispenser liner small amount liquid, appears possible seep leak on diesel union above shear valve, had JMP service visit that stated leak could not be seen, facility continues to monitor; South dock dispenser both piping donuts pushed-out of piping boot, sensor positioned right at piping invert, liner dry; shear valves anchored, hoses and hose reels good condition; emergency pump shut-off at South end of dock near kiosk.Pneumercator TMS 3000 (DEP EQ-303) with 1 tank interstice, 2 STP sump, 1 piping transition sump, and 3 dispenser pan sensors installed - all NORMAL at time of inspection; facility has daily sensor status tapes for the period 8/1/2009 through 7/15/2010 - all NORMAL; however all sensors except tank interstice not positioned. Facility has SWS agreement entered into with Nautico - advised Braden they would need to get one signed with the current owner/operator; spill plan and employee annual spill training sign-off could not be located. Sent e-mail to Braden 7/16 with spill plan template, sample visual inspection records and list of certified contractors that would have to be used if the transition sump is replaced. Advised that the Discharge Prevention and Response Certificate was a year overdue for being renewed and I can not issue until they have a current spill plan, response agreement and current employee annual training record. | ||||||||||

Description: | 2006_oct_20_rhs: Contractor is Petroleum Marine Consultants, Observed tanks set end-to-end, North-South allignment in excavation, not yet tied-down. 2006_dec_14_rhs: Site visit with Joe Zwissler, PCHD Observed South Mid-grade UL tank vacuum of 30" Hg and North Diesel tank vacuum of 26" Hg. Tanks are ballasted and back-filled to top of tanks. 2 SW spill buckets EBW 705-470-01; 2 vent lines are SW FRP Ameron Dualoy 3000L; 3 psi air pressure on vent lines with dual point vapor recovery and ball float valves installed Product piping is Environ DW 2" flex in corrugated chase; length of piping from Diesel STP sump to Western Fiberglass transition sump is 240 ft; length of piping from Mid-grade UL STP sump to transition sump is 170 ft; both lines are an additional 240 feet from the transition sump to the first dispenser, and then 70 ft more to the next termination dispenser.
