The installation inspection generated new violations, all for tests. When the tests are complete, send to me and I can resolve those violations. The Limited Closure report needs to be filled out and returned to me, I will enter it into the system. The registration needs to be filled out and returned to me, I will send to FDEP. I did not fill out Facility info, not sure if it should be kept as Bright House or changed. The Facility ID and tank information have been entered on the form. Call with any questions. The tank was manufactured by: ZBM Inc. (EQ-633) P.O. Box 486 Winnebago, MD 56098 Phone: (507) 893-3688 Serial #C-432424 Tank is equipped with a Zierkebuilt spill bucket, sight gauge for overfill protection, and an electronic interstitial float sensor. Alarm was operational at inspection. STRF was submitted to FLDEP. FINANCIAL RESPONSIBILITY: Chartis Specialty: 6/1/2011 ¿ 6/1/2012. Blank CFR provided for update. RELEASE DETECTION, as of 8/7/2011: RDRL was also provided at inspection. a. Monthly visual inspections which includes electronic interstitial monitoring of the double-walled tank. Inspections shall also include the tank exterior, spill containment, fill gauge, generator interior, and surrounding area. b. Annual operability test of the tank interstitial sensor.
