System manufactured by: Phoenix Products (EQ-013) Model # MH15322 S/N: 1701536 UL# C25154390 Manufacture Date 11/2018 Tank is equipped with a steel-fabricated spill bucket. System was shipped and delivered with vacuum, current reading is 12 and holding. The interstitial sensor was successfully tested at inspection. Next test will be due by 12/3/2019. RELEASE DETECTION, as of 12/3/2018: a. Monthly visual inspections which includes the tank exterior, spill containment and surrounding area. b. Electronic interstitial monitoring of the double-walled tank. File Review 12/31/2018: FINANCIAL RESPONSIBILITY: Current proof of Financial Responsibility has not yet been provided.
