Description: Met onsite with David Mooney (Spatco Technician) for this routine tank compliance inspection. A copy of this inspection report and enforcement referral notice letter will be emailed to Diane Rae at drae@bjs.com, Bill Peters at bpeters@bjs.com, and Sara Merritt at Sara.Merritt@FloridaDEP.gov. Underground Storage Tanks: Tank #1 and Tank #2 are 20,000 gallons Fiber Glass Systems, Double-Wall Fiberglass Tanks (EQ-191) containing Unleaded Gas. Tank #3 is a 24,000 gallons Fiber Glass Systems, Double-Wall Fiberglass Tanks (EQ-191) that is compartmented containing 12,000 gallons Premium and 12,000 gallons Diesel. All three tanks were installed October 2019. Facility has two unknown Aboveground Storage Tanks that appeared to be sub-base tanks that supply diesel fuel to an emergency generator. Our department was not notified about the installation of the Aboveground Storage Tanks. – Violation Issued. It was noted that the unregistered Aboveground Storage Tank System contains diesel fuel despite not having a current placard. The facility will be immediately referred to enforcement according to the Florida Administrative Code 62.762.401(5) -Violation Issued. Piping: 3" Franklin Fueling, UPP Double-Wall piping (EQ-816). (Vent piping) Ameron, Dualoy 3000/LCX, 2' Double-Wall fiberglass piping (EQ-291). Dispensers: Franklin Fueling Systems, PT Dispenser Liners (EQ-423). The dispenser sumps were mostly clean and dry at the time of inspection. The boots appeared to be in good condition. The hoses and nozzles appeared to be in good condition. STP Sumps: Franklin Fueling Systems, APT Sump Liner (EQ-423). The STP sumps were mostly clean and dry at the time of inspection. The boots appeared to be in good condition. The sensors were in the correct position. Spill Containments: Emco Wheaton Double Walled Secondary Spill Containment (EQ-741). The double wall spill containments were mostly clean and dry at the time of inspection. The gauges were normal at the time of inspection. Overfill Prevention: Emco Wheaton, Guardian Overfill Prevention Valves (EQ-414). Release Detection: The tanks interstitial sensors and STP sump sensors are monitored by a Veeder-Root TLS-450, with dual point, and non-discriminating sensors (EQ-724). DPLLD sensor (EQ-722), Universal Sensor/Probe Interface Module (USM), with water detection sensor. (EQ-723). Sensor status reports are printed monthly. All function normal on ATG at the time of inspection. Monthly Visual Inspection are being conducted by the facility. Records: All facility records were received for the underground storage tank system, including associated tank component testing, monthly visual inspection with electronic release detection ATG printouts, and financial responsibility forms and the current placard was posted at the facility. Kevin responded and couldn’t detect any significant odors using his equipment. I would, though, like for someone in your office to conduct an inspection to follow up and determine if there has been any unexplained loss of product from one of their tanks, and to review their ATG inventory/RD information. Given the large volume of petroleum stored at the facility and the lengthy underground piping, I believe it would be a good idea to check it out on the tank inspector side. The last routine inspection was in May 2021, and everything appeared in compliance. There are no other known contaminated areas around that site, nor are there any other fueling facilities nearby. Please follow up with an inspection tomorrow if possible. Conduct the inspection as fact finding to get a fix on if there has been any unexplained loss, but invoice it as a “complaint” inspection. Kevin would like to meet the inspector at the site during the inspection, so please give him a call at 850-258-4354 once you plan your time to be there. Description: The complaint inspection was conducted at BJs Wholesale Club Retail Gas ST, Club #382 by request of FDEP due a received report of petroleum odor in the sewer manways at a nearby location to the subject facility. Dispatched Operation Emergency Response FDEP Kevin Long noted no detectable significant odors with equipment on 06/14/2022. On 06/15/2022, Daniel Gartin conducted a review of the ATG inventory/RD and associated liquid status at the facility. The review of the facility's inventory and deliveries noted multiple deliveries can occur at the facility per day. These deliveries are most likely due to high consumer traffic that was observed at the facility than any unexplained product lost from associated facility's underground storage tanks. The review of the ATG liquid status and alarm history noted all functions were normal at the time of inspection. However, the Veeder Root TLS-450 noted an Sensor Out Alarm for the Regulat East Interstitial sensor and alarm history noted the same alarm, Sensor Out Alarm, for perm/dsl int on the previous day. It is noted that Liquid Status from the Sensor Status Report and Sensor Diagnostic - Liquid Sensor Information Report both resulted in Normal status with no indication of a sensor out alarm. Violation issued to facility to investigate why associated alarm history regarding Sensor Out Alarm is occurring at this facility.

Message: Good morning, The Florida Department of Health in Escambia County - Environmental Health Division is contracted with the Florida Department of Environmental Protection to conduct Storage Tank System Compliance Inspections at facilities located in Escambia County. The facility listed below are scheduled for a Tank Compliance Inspection on Friday, May 7th around 9:45am-10:15nn timeframe. Please contact the inspector if you have a conflict with the said schedule. Otherwise, please be ready for the inspection. FAC ID# 9817495 BJs Wholesale Club Retail Gas 7060 Plantation Rd, Pensacola, FL 32504 The tank owner/operator or a facility representative will need to accompany the inspector and provide access to all tank related equipment. Tank equipment may include: tank exterior, piping, piping sumps, spill boxes, overfill equipment, leak detector gauges, automatic tank gauges, dispenser hoses and dispenser sumps. Please be sure the tank equipment is ready for inspection. In addition, the following records need to be available for review: • Proof of Financial Responsibility or Proof of Insurance. Make sure it is in the State of Florida DEP Form 62-761.900(3) https://www.flrules.org/Gateway/reference.asp?No=Ref-11170 • Current Tank Registration Placard displayed in plain view at the site. • Maintenance and Repair Records for the last three years. • Monthly Visual Inspection Checklists for the last three years. • Monthly Electronic Release Detection for the last three years (as applicable). Check of Tank IM Sensor may be noted on Monthly Visual Inspection Checklist. • Testing Records (as applicable): Annual Operability Test Report for Tank Interstitial Monitoring Sensor/Release Detection Devices (e.g. Basin Alarm, Veeder Root, Krueger Leak Gauge, etc.); Annual Operability Testing for Overfill Protection Devices (e.g. Clock Gauge, Krueger Fuel Gauge, etc.). • For underground components (as applicable): Annual Line Leak Detector Test for underground pressurized piping and/or piping over surface water. ATG/Release Detection: Veeder-Root TLS-450, with dual point, and non-discriminating sensors.(EQ-724). DPLLD sensor(EQ-722), Universal Sensor/Probe Interface Module(USM), with water detection sensor. (EQ-723) Piping/Sump Fittings: Bravo, fiberglass, conduit pass-through fittings(EQ-820). Overfill Prevention: Four(4) Emco Wheaton, Guardian Overfill Prevention Valves(EQ-414) 10/25/2019 Tanks were delivered to the facility removed from trucks and set on beds of pea-gravel until ready for installation. Tanks appeared to be in good condition and did not have any signs of damage. Brine filled interstices were full and no leaks were seen on the tanks. 10/29/2019 Air pressure test were conducted on the tanks. Each primary was pressurized to 5 PSI, and was holding.

Description: Tanks were back filled using gravel. The STP sumps, fill pipes, and spill buckets were installed on the tanks. The dispenser sumps were installed in place. Piping was ran between the STP sumps and dispenser sumps. Piping was welded to connections at the dispenser sumps in accordance with the manufactures installation guidelines. Piping was pressurized with air, none of the fittings were seen leaking, or bubbling. All primary and secondary pipe air gauges were holding steady pressure. All dispenser sumps and STP sumps were seen passing hydrostatic test.
