(Perma-Fix), A Subsidiary Of Perma-Fix Environmental Services, Is A Large Quantity Generator And A Permitted Hazardous Waste Storage Facility. The Facility'S Processes Include Liquid And Sludge Bulking, Scintillation Vial And Other Small Container Crushing And Shredding, Repackaging Of Solid Wastes Contaminated By Hazardous Wastes, Stabilization Of Wastes In Containers, Thermal Desorption And/Or Chemical Oxidation, Used Oil Handling, Petroleum Contact Water (Pcw) Transportation And Consolidation And Storage Of Discarded Mercury-Containing Devices. Perma-Fix Was Issued Permit Number 17680-009-Hc On September 15, 2006. The Permit Is For The Operation Of A Hazardous Waste Treatment, Storage Facility Consisting Of A Tank, Two Container Storage Areas, And Two Miscellaneous Treatment Units. The Facility Was Last Inspected On June 16, 2008. Operations Have Not Changed Since Then And The Same Procedures Are In Place And Being Implemented. | Processing And Storage Building ----------------------------------------------- The Processing And Storage Building (Psb) Is A Permitted Hazardous Waste Container And Tank Storage Area, As Well As A Fuel-Blending Location That Includes Phase Separation Treatment. The Hazardous Waste Containers That Were Examined Were Properly Labeled And Closed And Had Been Dated With The Storage Start Date. Pallets Are Used For Stacking Drums A Maximum Of Two High, With A Maximum Of Four 55-Gallon Drums Per Pallet. A 3,000-Gallon Ast Is Located In The Approximate Center Of The Permitted Container Storage Area. The Tank Has Appropriate Secondary Containment And A Sump To Contain Any Spillage. At The Time Of This Inspection, The Ast Was Empty And Has Not Been Used To Store Any Waste Since It Was Installed. The Majority Of Waste Consolidation And Fuel Blending Occurs In The East Side Of The Permitted Tsd Building. Hazardous Wastes That Arrive In Drums Are Consolidated Into Tanker Trucks For Fuel Blending. Containerized Hazardous Wastes That Contain A High Percentage Of Solids Or Sludge Are Sometimes Blended With Liquids From One Of The 550-Gallon Totes To Obtain A Mixture That Can Be Pumped Into A Tanker Truck. The Mixture Is Then Manifested To Energis, Llc. Sludges/Solids Are Consolidated Into 55-Gallon Drums And Disposed Of As Hazardous Waste. All Containers Of Hazardous Waste In The Fuel Blending Section Of The Permitted Storage Area Were Properly Closed, Labeled, And Dated. Subpart Bb Equipment Was Properly Working At The Time Of Inspection. Treatment And Operations Building -------------------------------------------------- The Permitted Storage Area Has Seven Different Zones Of Secondary Containment Inside The Treatment And Operations Building (Tob). A Maximum Volume Of 35,200 Gallons Of Hazardous Waste Can Be Stored In The Tob As Stipulated In Attachment B Of The Permit. At The Time Of The Inspection The Facility Was Not Exceeding The Maximum Volume Or The Volume Limits For Each The Individual Storage Zones. The Perma Fix-I (Pf-I) And Perma Fix-Ii (Pf-Ii) Operations Occupy The Northeast Portion Of This Building. Both Pf-I And Pf-Ii Processes Are Conducted In A Quonset Hut That Is Located In This Building. The Pf-I Process Consists Of Stabilization And Fixation Treatment. This Is Conducted Primarily In 55-Gallon Drums. The Pf-Ii Process Consists Of Thermal Desorption And/Or Chemical Oxidation Batch Treatment Of 110-Gallons Or Less. Perma-Fix Analytical Services Occupies The Southeast Corner Of The Tob. The Laboratory Performs Fingerprint Analyses On Each Incoming Waste Stream To Confirm That Waste Received By The Facility Conforms To The Initial Waste Profile. Laboratory Operations Generate Waste Acid, Solvents, Liquids, Glass, And Plastic That Are Managed As Hazardous Waste And Are Transferred To The Lsf Processing Area For Liquid Recovery. Waste Glass And Plastic Are Processed Through The Lsf Area And Are Shipped To Covanta As Non-Ha

Treatment And Operations Building (Tob): At The Time Of The Inspection The Facility Was Not Exceeding The Maximum Volume (35,200 Gallons) Or The Volume Limits For Each Of The Individual Storage Zones. The Perma-Fix I (Pf-I) And Perma-Fix Ii (Pf-Ii) Operations Occupy The Northeast Portion Of This Building. Perma-Fix Analytical Services Occupies The Southeast Corner Of The Tob. The Laboratory Performs Fingerprint Analyses On Each Incoming Waste Stream To Confirm That Waste Received By The Facility Conforms To The Initial Waste Profile. Laboratory Operations Generate Waste Acid, Solvents, Liquids, Glass, And Plastic. The Laboratory 90-Day Accumulation Area Is Located Just Outside The Lab In The Middle Portion Of The Tob. At The Time Of The Inspection, There Were Three 55-Gallon Drums That Were Properly Labeled, Dated, And Closed. In The Northern Portion Of This Building Was A Satellite Conatiner For Spent Aerosol Cans. This Container Was Not Closed Or Labeled [40 Cfr 262.34(C)(1)(I) And 262.34(C)(1)(Ii)]. This Was Corrected During The Inspection. Lsv Processing And Waste Storage Building The Waste Storage Warehouse (Wsw) Contains A Newly Permitted Hazardous Waste Container Storage Area, Liquid Scintillation Fluid (Lsf) Processing Area, Debris Treatment Unit, Maintenance Shop, And A Separate Storage Area For Used Oil, Oily Wastewater, And Other Non-Hazardous Wastes. The Hazardous Waste Containers That Were Examined Were Properly Labeled And Closed, And Had Been Dated With The Storage Start Date. At The Time Of The Inspection, The Facility Was Not Exceeding The Maximum Volume Or The Volume Limits For The Storage Zones. Spent Mercury-Containing Lamps Were Being Stored On The East Side Of The Wsw Building. Spent Mercury-Containing Lamps Are Shipped To Aerc, Inc. For Reclamation. The Accumulation Area Was

(Perma-Fix) Was Inspected On June 23, 2011, By The Department Of Environmental Protection (Dep) Hazardous Waste Section As An Unannounced Compliance Inspection. Yasmine Enriquez And Scott Jonhson Of Dep'S Air Section Were Also Present During The Inspection. Perma-Fix Is A Large Quantity Generator And A Permitted Hazardous Waste Storage Facility. The Lsf Contain Small Amounts Of Xylene And Toluene And May Be Radioactive. The Lsf Is Therefore Regulated Under The Resource Conservation And Recovery Act (Rcra) As F003/F005 Hazardous Waste, And, If Radioactive, As A Mixed Waste Under Rcra And Nuclear Regulatory Commission (Nrc) Rules. At The Time Of Inspection, The Unit Was Not In Operation, But Was Last Operated The Previous Week. Maintenance Shop The Maintenance Shop Is Located In This Building And Contains One Parts Washer. Diesel Fuel Is Used As A Solvent, Generating A Non-Hazardous Waste Parts Washer Fluid. The Facility Generates Less Than 55 Gallons Per Year Of This Waste Stream, Which Is Added To The Used Oil In The Fuel Blending Area For Energy Recovery As Needed. The Facility Receives, Temporarily Stores And Transfers Radioactive And Mixed Wastes In Accordance With Us Nuclear Regulatory Commission (Nrc) And Rcra Requirements. The Facility Operates Monday Through Friday From 5 A.M. To 4 P.M. And Is Connected To City Water And Sewer. Containers Are Received At The Facility, Inspected, Cross-Checked With The Manifest, Labeled With Perma-Fix Inventory Labels, Logged Into The Facility’S Electronic Inventory System And Then Placed Into The Appropriate Storage Area. There Are Three Storage Areas In The Facility Where Containers Are Stored Prior To Treatment, Blending Or Off-Site Shipment. These Areas Are Described Below. Unless Otherwise Stated Below, All Waste Accumulation Containers Observed Were Closed, Properly Labeled And Dated. | Processing And Storage Building (Psb) --------------------------------------------------------------------------------------------- The Area Is Not Enclosed But Is Under A Roof And Has A Containment System And Concrete Curbing With Three Different Zones For Storage Of Waste. This Storage Area Is Permitted To Store Up To 72,105 Gallons Of Hazardous Waste With A Maximum Container Size Of 718 Gallons That Is Equivalent To A B-25 Box Container. At The Time Of The Inspection, This Storage Area Was Within Its Permitted Volume Capacity And All Containers Had Been Accumulating For Less Than One Year As Documented In The Facility’S Electronic Inventory System. Drums Are Moved To Different Zones Within The Building For Storage And Treatment As Appropriate. The Following Activities Occur In This Area: Fuel-Blending Including Phase Separation Treatment, Decanting And Bulking Of Wastes; Lab-Pack Decommissioning; And Bulking Of Chemotherapy And Pharmaceutical Wastes. The Building Was Near Capacity And Most Of The Drums Were Stacked Two High (Photo 1), Aisle Space Was Tight And A Few Of The Drums Observed Were Not In Good Condition (Photo 2). It Was Recommended To The Facility That Drums May Need To Be Over Packed If The Condition Is Questionable Or If Waste Is On The Outside Of The Drum. The Facility Was Also Reminded About Maintaining Adequate Aisle Space Around The Drums.

(Perma-Fix, The Facility) Was Inspected On June 24, 2021. A Site Visit Was Also Conducted On August 31, 2021, For Records Review. Mr. Randy Self, General Manager (Perma-Fix) Was Present During June Inspection. Perma-Fix’S Mr. David Fendelander (Ehs Manager), Mr. Lason Kirkland (Industrial Operations Manager), And Mr. John Cochran (Transportation Manager) Were Present During The August Records Review. Perma-Fix Was Last Inspected Jointly By The Department’S Hazardous Waste Program And U.S. Environmental Protection Agency On November 21, 2019. Perma-Fix Was Issued Rcra/Hswa Permit Number 17680-011-Ho On June 3, 2020, With An Expiration Date Of June 8, 2025. The Permit Is For The Operation Of A Hazardous Waste Treatment And Storage Facility Consisting Of A 3,000-Gallon Aboveground Storage Tank (Ast), Three Container Storage Areas And Three Miscellaneous Treatment Units. Perma-Fix Is A Large Quantity Generator (Lqg) Of Hazardous Waste, A Used Oil - Used Oil Filter Transporter And Transfer Facility, A Universal Waste (Lamps, Batteries, Devices, Aerosol Cans) Transporter And Transfer Facility, And A Used Oil Marketer. The Facility'S Processes Include Liquid And Sludge Bulking, Scintillation Vial And Other Small Container Crushing And Shredding, Repackaging Of Solid Wastes Contaminated By Hazardous Wastes, Stabilization Of Wastes In Containers, Thermal Desorption And/Or Chemical Oxidation, Non- Elementary Neutralization, Used Oil Handling, Petroleum Contact Water (Pcw) Transportation, Consolidation And Storage Of Universal Wastes, Radiological Screening, And Chemical Analyses Of Wastes. The Facility Receives, Temporarily Stores And Transfers Radioactive And Mixed Wastes In Accordance With Us Nuclear Regulatory Commission (Nrc) And Rcra Requirements. Perma-Fix Has Approximately 45 Employees At The Facility And Operates Monday Through Thursday From 6 A.M. To 4 P.M. And ½-Day On Friday. The Facility Is Connected To City Water And Sewer. Containers Are Received At The Facility, Inspected, Cross-Checked With The Manifest, Labeled With Perma-Fix Inventory Labels, Logged Into The Facility’S Electronic Inventory System And Then Placed Into The Appropriate Storage Area. There Are Three Storage Areas In The Facility Where Containers Are Stored Prior To Treatment, Blending Or Off-Site Shipment. These Areas Are Described Below. Unless Otherwise Stated Below, All Waste Accumulation Containers Observed Were Closed, Properly Labeled And Dated. | Processing And Storage Building (Psb) The Area Is Not Enclosed But Is Under A Roof And Has A Containment System And Concrete Curbing With Three Different Zones For Storage Of Waste (Photos 1 And 2). This Storage Area Is Permitted To Store Up To 72,105 Gallons Of Hazardous Waste With A Maximum Container Size Of 718 Gallons That Is Equivalent To A B-25 Box Container. At The Time Of The Inspection, This Storage Area Was Within Its Permitted Volume Capacity And All Containers Observed Had Been Accumulating For Less Than One Year. Drums Are Moved To Different Zones Within The Building For Storage And Treatment As Appropriate. The Following Activities Occur In This Area: Fuel-Blending Including Phase Separation Treatment, Decanting And Bulking Of Wastes; Lab-Pack Decommissioning; And Bulking Of Chemotherapy And Pharmaceutical Wastes. An Aisle Near The Middle Of The Psb Separates Zones 1 And 2 From Zone 3. Containers Are Staged Here For Initial Sampling. Zone 1 And A Portion Of Zone 2 Are Used To Stage Containers For Bulking Of Liquid Hazardous Wastes. The Facility Pumps The Liquid Waste From The Drums Via An Overhead Pumping/Dispensing System Into Poly Or Stainless Totes Or Tanker Trucks. Bulking Of Waste Usually Occurs The Later Part Of The Week To Allow For Monday Or Tuesday Shipment.
