02/23/2017 Site Inspection Routine; Used Oil Transporter A Routine Compliance Evaluation Inspection (Cei) Was Conducted At Havana Waste Oil Services Inc. (Hwos) On February 23, 2017. Facility Operates As A Used Oil Transporter (Uot), Under The Ownership Of Roberto Gonzalez Frias (President) And Mrs. Yisandra Ross (Vice-President) . The Facility Is Located In The Home Of Mrs. Ross (Recordkeeping) And Truck Location Is In The Industrial Parking Lot Of Opa Locka Flea Market. The Business Was Purchased By Mr. Gonzalez On June 2016. Mr. Gonzalez Is The Owner And Transporter. According To The Documentation Provided, The Facility Is Currently Registered As Used Oil And Used Oil Filters Transporter With The Department Effective On 07/25/16; Expiration Date 6/30/2017. Notification History: 06/16/2016 - Facility Notified As Uot Under Their New Ownership Inspection History: Never Inspected By The Department But They Have An History Of Non-Compliance (Enforcement) With Their Previous Ownership Havana Waste Oil, Inc. On 6/18/14. Four Violations Were Noted During That Inspection And Facility Returned To Compliance With Enforcement. | Hwos Has One 2,500 Gallons Capacity - Tanker Truck Also Capable Of Transporting Used Oil Filters Drums. The Facility Parks Its Transport Vehicle Located At The Industrial Parking Lot Of The Opa Locka Flea Market. This Parking Area Completely Enclosed With 24-Hour Security, And Tenants Have Access Using Remotes. Dot Placards Were Displayed On The Side Of The Truck Model Freightliner 1998 And The Transporter Keep Copies Of The Permits, Notifications And Manifest On The Truck. Inspectors Observed That Hwos Did Not Have Copy Of Their Current Dot Registration. Also, The Inspectors Observed The Truck Was Equipped With Halogen Screening Tools And Spill Kits For Transportation-Related Response Incidents. The Truck Was Also Equipped With Two Fire Extinguishers; The Extinguishers Were Not Current With Their Annual Inspection. According To Mrs. Ross Hwos Collects Used Oil, Used Oil Filters, And Spent Antifreeze From Cesqg'S And Their Tanker Truck Delivers Directly To The Following Permitted Used Oil Processors: Us Lubricants (Flr000211557), Raider Environmental (Flr000143891) And Ricky'S Oil (Epa Id: Fld981019755). Hwos Is Not A Registered Marketer Or Transfer Facility. Record Review: A. Disposal Records Used Oil/Used Oil Antifreeze/Used Oil Filters: Pick-Up And Delivery Records Were Provided At The Time Of Inspection. Transport, Receiving And Shipping Records Were Available For Review At The Time Of Inspection. The Facility Was Using An Equivalent Form [62-710.901(2)]. Each Acceptance And Delivery Records Comply With The Requirements Described In 40 Cfr Part 279.46. Acceptance And Delivery Records Displayed Testing Procedures Before And After Pickup Of Used Oil Using A Tif 8800 Testing Unit For Total Halogen Content. The Inspectors Observed That Most Of Their Generators Are Cesqg'S And Documentation Appeared To Be In Order Except For Some Of The Delivery Records Failed To Included Hwos Epa Id Number. B. Insurance Liability - The Facility Provided Proof Of Pollution Liability Insurance (In The Amount Of $1m) Policy - Global Insurance And Financial Services Policy #73trs066065 Exp. Date: 7/13/2017 C. Employee Training - Documentation Was Not Available For Review During The Inspection. Also, Hwos Will Submit Their Annual Report For Used Oil Activities Before March 1, 2017. Related To Hazardous Wastes, This Facility Is A Non-Handler. | The Facility (Uot) Was Out Of Compliance At The Time Of The Inspection. The Facility Was Given 15 Days To Return To Compliance. On 3/9/17, Hwos Provided The Following Documentation: Delivery Records Including Their Epaid Number, Employee Training Logs And Dot Registration. Facility Returned To Compliance On 3/10/17.
